Intramedullary astrocytoma spread three and a half years after surgery

Patient: examination and laboratory tests: just made the film: affects the performance: lower thoracic spine postoperative, thoracic 10-12 segmental vertebral body posterior tissue structure disorder. The thoracolumbar vertebrae are seen to be aligned as usual, with osteophytic changes visible at the vertebral margins and abnormal signal in the thoracic 9-lumbar 1 vertebrae, presenting slightly higher signal shadows in T1W and T2W relative to the other vertebrae. The spinal cord was thickened in the thoracic 5-waist 1 segment, with abnormal intramedullary signal, slightly low signal such as T1W on plain scan and equal high signal in T2W, and enlarged heterogeneous enhancement was seen after enhancement. The spinal cord cone structure was not clearly shown, and the morphology and signal of the cauda equina nerve shown were not significantly abnormal. Treatment: April 14, 2009, neurosurgery in Shanghai Huashan Hospital, followed by radiotherapy, and several times of Chinese medicine in between. In the summer of 2011, he started to have weakness in his legs, which became more and more serious, and he couldn’t walk in the fall. I’m not sure if it’s a good idea to have another surgery, because it’s a long one. The main thing is decompression surgery. Is it possible to operate again, because it is longer, and can all of it be removed? Decompression surgery can be considered, but the nerve function is still not expected to be restored. Patient: Thank you, Dr. Yuan, for your reply, now is the time to remove all the tumor, is the surgery risky? Can I save my life? Is the tumor growing fast now? I’m looking forward to your answer, thank you. Yuan Ge, Department of Neurosurgery, Beijing Tiantan Hospital: Is it risky to remove all the tumors? It is impossible to achieve. Can we save our lives? Is the tumor growing fast now? Life is not a concern unless there are serious comorbidities. The patient: But the doctor said that if the tumor continues to grow, it will be difficult to breathe and affect the life, is it possible to remove the nerve and tumor together with the spinal cord? It can’t be reached at one time, but it is difficult to say about the comorbidities. Therefore, there is still some justification for active treatment. Patient: Thank you very much. We’ll get ready to go to Beijing as soon as we can, and then we’ll go to your number.
