In the past decade or so, the incidence of neck pain among adolescents has been increasing. According to preliminary statistics, the age of onset of neck pain is mostly concentrated in 12-13 years old in elementary school students, 16-18 years old in junior high and high school, and usually 22-24 years old in college students. It can be seen that its onset has a greater relationship with graduation and promotion examination season. The reason for this is that poor posture in learning and living is one of the main causes of neck pain. Experts point out that many parents are so eager to see their children become dragons and phoenixes that they only pay attention to their children’s studies and neglect their children’s daily health care. According to relevant survey data, university, middle school and elementary school students in the incorrect posture of ambulatory learning accounts for about 70%-80% of the total. Or a long time lying on the head to read books, writing, crooked head, end shoulder; or love to lie on the sofa to read, watch TV; or play with the computer for too long, or addicted to the “Internet bar”, etc., all kinds of bad posture leads to the cervical vertebrae for a long time in a flexed position or some specific position, the neck is in excessive forward extension and forward flexion, so that the front of the neck muscles, ligaments and other soft tissue Long-term tension contracture state, the back of the neck muscles, ligaments and other soft tissues are strained strain; and then cause the cervical intervertebral disc, vertebral body small joint structure relaxation instability, coupled with twisting, lateral flexion excessive activities, affecting the cervical spine itself and the surrounding nerve, vascular tissue structure, the emergence of neck pain disease. The symptoms include neck pain, restricted movement, pain radiating to the shoulder and arm, or numbness in the arm; sometimes accompanied by headache and dizziness, nausea and vomiting, loss of appetite, fullness and distension in the chest and abdomen; or accompanied by mental depression, impatience, loss of vision, drowsiness or insomnia, dreaminess and forgetfulness, lack of concentration, and decreased academic performance. Although the above symptoms can be relieved by rest, the pathological changes of the cervical spine, if not corrected, will eventually evolve into cervical spondylosis, which will cause lifelong damage. Palace osteopathy originated in the Qing Dynasty at the Palace of the Qing Dynasty, and was a medical technique used by the Qing Dynasty court doctors to treat bruises and joint dislocations, and was a purely Chinese medical treatment system based on manual treatment, supplemented by Chinese herbal medicine and Chinese medical instruments. Sun Shuchun studied under Mr. Liu Shoushan, a famous orthopedic surgeon in the capital and an heir to the palace orthopedic art, and received the true transmission of the palace orthopedic art. As a student of Prof. Sun Shuchun, I have learned from him that the “lightness, dexterity, flexibility and transparency” of his palace orthopedic techniques are very suitable for the treatment of neck pain in adolescents. Using Sun Shuchun’s Gongting Bone Setting Technique, he treated nearly 200 cases of neck pain in patients aged 15 to 25 years old and achieved good results. Not only did the patients get relief from their symptoms quickly, but the cervical spine X-ray examination confirmed that Sun’s Gong Ting Orthopedic Technique can also correct the physiological curvature, vertebral rotation and other pathological changes of the adolescent cervical spine, which has the special effect of treating both the symptoms and the root cause of adolescent neck pain. Since the symptoms of adolescent neck pain are similar to those of cervical spondylosis, some people call it “adolescent cervical spondylosis”. Cervical spondylosis refers to a chronic common disease caused by degenerative lesions of the cervical disc and its secondary osteophytes, which compress the surrounding blood vessels, nerves and spinal cord and cause clinical symptoms, mostly occurring in middle-aged and elderly people. In recent years, the incidence of cervical spondylosis in adolescents has been increasing, and some people have calculated that it accounts for about 20% of cervical spondylosis. As the intervertebral discs of adolescents have not yet degenerated or are in the initial stage of degeneration, there is often no obvious manifestation of cervical bony metamorphosis on X-ray examination, only straightening of the physiological curvature or limited instability and displacement of several vertebrae, “bilateral signs” in the adjacent upper and lower joints and the posterior edge of the vertebral body, and oblique spinous processes on orthoptic films, and even a few patients do not have X-ray changes. A few patients do not even have X-ray changes. The disease is fascial and does not penetrate deeply into the bones. From the perspective of disease regression, adolescent neck pain is reversible with proper treatment and change of poor postural habits. Therefore, adolescent neck pain does not match the definition, pathological changes and disease regression of cervical spondylosis, and should not be placed in the category of “cervical spondylosis”. Neck pain in adolescents is a preliminary manifestation of cervical spondylosis, and cervical imbalance is the key pathological change, and it is reversible, so it should be called “cervical imbalance syndrome”, and its pathogenesis is due to chronic injury of neck muscles, imbalance of muscle tension on the symmetrical side of the cervical spine, destruction of the physiological structure of the cervical spine and imbalance of mechanical balance, which in turn In addition to neck and shoulder pain, clinical manifestations include headache, dizziness, dry eyes, loss of vision, insomnia, memory loss, concentration problems, etc. The clinical manifestations are very complicated, which not only seriously affects the physical and mental health of adolescents, but also hinders the quality of life, learning and work, and if timely and effective treatment is not provided at this stage, pathological changes such as degenerative disc disease and its secondary osteophytes will occur in advance, leading to cervical spondylosis. eventually lead to cervical spondylosis. Therefore, correction of neck pain in adolescents is the best time for early intervention to prevent the occurrence of cervical spondylosis. For the treatment of neck pain in adolescents, conservative methods such as bed rest, peri-collar braking and traction are often used, while surgery can be used for those who cannot recover. However, surgery is very traumatic and expensive, and in the long term it may destroy the normal anatomy of the neck. The use of Chinese medicine massage and tui-na treatment, although there is a certain degree of efficacy, but because of the pathogenesis of the disease is not understood, so the massage techniques are not targeted, blindness, and even medical injury. Sun’s palace orthopedic techniques are based on the theory of “The Golden Guide to Medicine – The Essentials of Orthopedic Heart Method”, and the techniques strive to use excellence. In the treatment of neck pain in adolescents, we first analyze the clinical characteristics and imaging manifestations of patients and study the correlation between clinical characteristics and imaging. According to the principle of discriminative treatment, the imbalance of the cervical spine is analyzed and the principle of holistic Chinese medicine treatment is determined. Sun’s Gongting tendon manipulation was used to carefully identify and apply pushing, holding, pressing, kneading, rolling and plucking techniques to adjust the tension of soft tissues on the symmetrical side of the cervical vertebrae, so that the spasm was relieved and the spasm was reopened to restore the balance of soft tissue tension in the cervical vertebrae. The cervical vertebrae are then rotated and corrected by Sun’s palace orthopedic method, so that the wrong ones return to their position and the curved ones return to their original position; then the cervical imbalance of the patient’s meridian muscle tension and cervical vertebrae biomechanical balance can be restored, effectively interrupting the pathological evolution process of cervical imbalance developing into cervical spondylosis. Chinese medicine orthopedic manipulative treatment is a non-invasive safe, effective and green treatment method that the majority of patients with bone injuries are willing to choose. The palace orthopedic technique is the best of the Chinese orthopedic techniques, which is the essence of the centralized orthopedic techniques and was refined in the palace. Our teacher, Professor Sun Shuchun, not only inherited the palace orthopedic technique, but also further developed it in long-term clinical practice, forming the Sun’s orthopedic technique with a unique style. In the process of learning Sun’s orthopedic technique, I often perceived its subtlety and used it to treat neck pain in adolescents, and I felt that the characteristics of Sun’s palace orthopedic technique were “light, dexterous, spiritual and transparent”, and that patients could be effectively treated without knowing their suffering.
