Gout Treatment Myths

Mr. Wang, 52 years old, has a history of gouty arthritis for more than 10 years. He took colchicine tablets or painkillers for every painful attack and then left it alone after the symptoms were relieved, and never took regular uric acid-lowering treatment. He was diagnosed with chronic gouty arthritis, chronic renal insufficiency and bilateral kidney stones. Gout is an intractable and curable disease. It is a pity that the patient did not pay enough attention and did not take standardized uric acid-lowering treatment during the interval, which eventually developed into chronic gouty arthritis, kidney stones and impaired kidney function! With the improvement of living standards, the incidence of hyperuricemia and gout has increased year by year, and has become the second largest metabolic disease after diabetes, but we generally have a lack of awareness of the dangers of gout, and there are still many misconceptions in the treatment of gout. 1, long-standing disease into a good doctor, each gout attack namely according to the instructions to start taking colchicine tablets for treatment, the first time 1mg, and then 0.5mg every hour until the total amount reaches 6mg, or there is diarrhea or joint swelling and pain relief. Since the therapeutic amount of colchicine is very close to the poisonous amount, and the side effects are great, in fact, we do not advocate this way of taking colchicine nowadays, and if we need to take it, we should also take small doses of 1mg for the first time, 0.5mg after 1 hour, and 0.5mg every 12 hours afterwards. The effect is similar to that of high doses, but the side effects are greatly reduced. 2, every gout attack that go to treatment, no pain, regardless: gout is a chronic progressive disease, in addition to causing joint swelling and pain will also cause damage to the kidneys, cardiovascular and cerebrovascular. It can cause interstitial lesions of the kidneys, which can eventually lead to uremia; long-term hyperuricemia can also cause arteriosclerosis, stroke and myocardial infarction. Therefore, we need to pay attention to the standardized uric acid reduction treatment during the interval, only when the blood uric acid is controlled, we can fundamentally control the re-emergence of gouty arthritis and prevent the related diseases caused by hyperuricemia. 3, excessive emphasis on diet control: human uric acid consists of two parts: endogenous and exogenous uric acid, endogenous uric acid is produced by the body’s metabolism, accounting for 80% of the total uric acid level, exogenous uric acid accounts for 20%, even if the diet is strictly controlled can only solve 20% of the uric acid level, which is equivalent to about 60-90umol/L blood uric acid level, so we emphasize diet control, but in addition to Therefore, we emphasize diet control, but in addition to diet treatment, we also need to receive some uric acid-lowering medication. 4.Other: Some patients with gout stone formation believe in surgical treatment, in fact, surgical treatment cannot solve the problem at the root, if the blood uric acid level is still high, gout stone will still be formed, and the surgical incision is difficult to heal. If the uric acid is reduced to below 300umol/L, the gout stone can be softened slowly and even absorbed. In addition, we remind you not to buy some proprietary Chinese medicines for gout, which often contain hormones, and recommend going to a regular hospital for standardized treatment.
