Oral cancer is the most common malignant tumor in oral and maxillofacial area, including malignant tumor of tongue, cheek, gum, palate and floor of the mouth, especially tongue cancer, which is often a medium or highly differentiated squamous cell carcinoma, prone to early lymph node metastasis in the neck, but less metastasis in distant organs. The modern treatment concept of oral cancer is comprehensive treatment mainly based on surgery, and the cure rate can reach 60-70%, and even 90% for early surgery. Relying on the overall advantages of a comprehensive hospital, the Department of Oral and Maxillofacial Surgery of our hospital vigorously carries out multi-disciplinary integrated treatment of oral cancer with surgery as the main treatment, supplemented by radiotherapy and chemotherapy, and the cure rate has reached the same level in China. Typical case: Mo×, female, 57 years old, was admitted to the hospital with a progressive growth of right tongue mass with pain for several months. MR showed a 3.7×2.8×2.0 cm swelling in the right tongue, crossing the midline of the tongue body, and multiple lymph nodes in the deep neck. The largest lymph node was 1.5 cm in diameter, which was considered to be a malignant tumor of the right tongue. Biopsy of the right tongue mass showed: highly differentiated squamous cell carcinoma. The clinical diagnosis was: right tongue cancer (T2N1M0). We formulated a comprehensive treatment plan of preoperative adjuvant chemotherapy + surgery + postoperative radiotherapy for the patient. The patient was given chemotherapy with pinyamycin + methotrexate + cisplatin for one week, and the primary foci of the tongue were significantly reduced after the chemotherapy, followed by enlarged resection of right tongue cancer + partial resection of right mandible + radical lymphatic dissection of right neck + tongue reconstruction with subchin island flap. One week later, the wound healed and was transferred to radiotherapy department for postoperative radiotherapy. The concept of comprehensive treatment, reasonable surgical selection, effective means of defect repair, necessary postoperative adjuvant treatment and consistent case follow-up are the keys to improve the cure rate and survival quality of oral cancer patients.
