Why some patients develop diarrhea after cholecystectomy

  At present and for a long time in the future, cholecystectomy, especially laparoscopic cholecystectomy, is still an effective clinical treatment for gallbladder stones and gallbladder polyp-like lesions. Most of the patients’ previous discomfort symptoms are obviously relieved after gallbladder removal, and the treatment effect is good. A few patients can have some discomfort symptoms, such as abdominal distension, diarrhea and increased number of stools in some patients after surgery, what are the reasons?  This has to be explained in terms of the function of the gallbladder. The main function of the gallbladder is to store and concentrate bile. Bile is secreted from liver cells, and the gallbladder does not secrete bile. The bile secreted from liver cells can be stored temporarily in the gallbladder, and the mucous membrane of the gallbladder absorbs some water in it to make the bile concentrated, and when the body eats, the concentrated bile in the gallbladder enters the intestine to participate in digesting food. Therefore, the reason why some people change the regularity of stool and increase the number of times or even diarrhea after gallbladder removal is mainly because: 1. There is no gallbladder to store bile temporarily, while the liver secretes bile uninterruptedly, so that bile enters the intestine continuously. The bile acid in bile has the function of promoting intestinal peristalsis, and the number of stools increases accordingly after the peristalsis of the gastrointestinal tract is enhanced.  2, the amount of bile in the human body is large, amounting to 500-700 ml per day, and the intestinal reabsorption may not be able to keep up after these bile continuously enters the intestine. When bile containing bile salts enters the colon, bile salts can stimulate its mucous membrane to secrete water and electrolytes, so it can cause diarrhea.  3. After the number of stools increases or diarrhea occurs, too much bile acid is lost, resulting in impaired digestion and absorption of fat, which further aggravates diarrhea.  These gastrointestinal discomfort symptoms after cholecystectomy can be slowly relieved, the length of time varies from person to person, some patients can be significantly relieved in 1-2 months after surgery, some patients last relatively longer, half a year or more than half a year. This mainly depends on the speed of compensation of the common bile duct after cholecystectomy and the difference of gastrointestinal function of each person. Patients with good gastrointestinal function rarely have the above symptoms after cholecystectomy or recover quickly after the symptoms appear.  The main ways to avoid bloating, diarrhea and increased number of stools after cholecystectomy are: 1. Pay attention to diet, eat less and more meals, and eat less food containing high fat and stimulating food.  2, more activities after surgery to enhance gastrointestinal function.  3, when the symptoms are obvious, oral antidiarrheal drugs, etc.
