Gout – the devil bites the foot

In ancient times, gout was called “imperial disease” or “disease of wealth”, and the earliest Westerners thought that gout was “the devil biting the foot”. It is characterized clinically by hyperuricemia, recurrent acute arthritis, gout stone deposition, chronic arthritis and joint deformity, substantial renal lesions and uric acid stone formation. Experts believe that control of the mouth (low purine diet), open legs, weight control and drinking more water can effectively prevent and treat gout. The discovery of high uric acid is a precursor Zheng often calls friends to get drunk on a regular basis, for six months, he often woke up at night with a painful heartburn, always waking up to find red and painful joints somewhere in his hands and feet, sometimes so painful that he would sweat profusely. The physician eventually diagnosed gout, and the pain was controlled by treatment. However, Zheng Mouqiang forgot about the pain and often forgot to take his medication. Until two weeks ago, he limped to the hospital for a follow-up, saying that his joint swelling and pain had flared up again. Experts remind that the underlying condition of gout disease is hyperuricemia, each person’s body will produce new uric acid every day, while excreting some of it to ensure that the body’s uric acid level balance. However, if there is too much uric acid in the body or if it is not excreted in time or if the uric acid excretion mechanism is degraded, supersaturated blood uric acid will be deposited in the joints and kidneys, causing gout. In the early stage of gout, most patients show monoarthritis during the acute attack, i.e., abnormalities in the first metatarsal and toe joints, followed by gradual appearance of redness, swelling, heat, pain and restricted movement in other joints of the ankle, hand, wrist, knee, elbow and foot. Treatment Regular medication is required “The primary goal of treatment is to reduce joint pain and swelling. When pain flares up, patients can rest the affected joint on their own and apply ice to the affected joint. Experienced patients can always keep some pain medication on hand and take it as soon as they notice discomfort in the joint, often stopping the progression of arthritis.” When you go to the hospital for treatment, medication should be given in adequate amounts of non-steroidal anti-inflammatory drugs, small doses of colchicine, and moderate amounts of glucocorticoids. It is understood that there are also intermittent episodes of gout. The absence of effective treatment after the first attack can lead to an increase in the frequency of attacks, a shortening of the interval, an increase in symptoms, a longer duration of inflammation and an increase in the number of joints involved. Most second attacks occur between six months and two years after the initial attack, with some individuals experiencing a second attack 27 years later. “The goal of this phase of treatment is to control blood uric acid and prevent gout attacks and their complications.” During the interval period, patients are not in discomfort, but uric acid is still wreaking havoc in the body. If left untreated, gout attacks will become more frequent, and in time, gout stones, joint destruction deformities, complications of uric acid nephropathy, urinary stones and eventually renal insufficiency will occur. As long as you take uric acid-lowering drugs regularly and control blood uric acid, this will prevent recurrent gout attacks and prevent complications from occurring. At present, there are clinical tests for blood uric acid, uric acid, synovial fluid, and x-ray. In the clinic, some patients are afraid to even eat meat, resulting in a monotonous and boring diet, and even malnutrition. In fact, rheumatology departments of large hospitals can provide a table of purine content of food or exercise and diet precautions for gout patients, such as open your legs, keep your mouth shut (low purine diet), control your weight, drink more water and other general knowledge for disease prevention. Exercise is not only good for gout patients, but also beneficial for reducing and relieving joint pain, preventing joint contracture and muscle wasting atrophy. There are many suitable items, such as brisk walking, jogging, tai chi, radio exercises, walking, qigong, shuttlecock, swimming, rope skipping, etc. Gout patients should choose exercise according to their physical condition, physical strength and endurance as well as personal preference. The time can be gradually increased from 15 minutes to 45 minutes, generally 5 times a week. Experts remind that the gout prevention diet should pay attention to the “amount”, try to avoid the intake of high purine food. As a gout patient, whether yellow wine, beer, white wine are not appropriate to drink, can be appropriate to drink some wine to cheer or light tea instead of wine, etc.. In order to prevent the accumulation of uric acid in the kidneys, you should drink a lot of water, the daily water consumption should be maintained at more than 2000 ml, and at the same time should drink evenly, do not drink too much at once.
