However, about 5% to 18% of patients will have diarrhea after surgery, which is characterized by an increase in the number of stools, unformed or even watery stools, and diarrhea will be aggravated by eating foods with high fat content. The occurrence of diarrhea after cholecystectomy is not related to the surgical method of cholecystectomy (traditional open surgical resection versus laparoscopic resection). I. Causes The gallbladder is an organ that stores and concentrates bile. The liver produces about 700 to 1000 ml of bile every day, which is first stored in the gallbladder through the bile ducts and concentrated 5 to 10 times by the gallbladder, and then the concentrated bile is discharged into the duodenum via the common bile duct by the contraction of the gallbladder during feeding to help digestion and absorption of fat. The causes of diarrhea after cholecystectomy may be as follows: 1. After cholecystectomy, a large amount of bile produced by the liver will directly and continuously flow into the duodenum, which will lead to malabsorption of bile acids and excessive entry into the colon. 2, too much bile acid into the colon can also stimulate the colon peristalsis, accelerate the intestinal contents through the colon, thus aggravating diarrhea. A study showed that the time for intestinal contents to pass through the colon in patients after cholecystectomy was shortened from 51 hours to 38 hours. 3. Due to excessive loss of bile acids, the concentration of bile acids in the intestine is not enough when eating foods with high fat content, which can lead to impaired digestion and absorption of fat and aggravate diarrhea. II. Treatment Patients with diarrhea after cholecystectomy should have comprehensive treatment from the following aspects 1, to maintain an optimistic state of mind, to understand that this is a normal phenomenon, to avoid tension, anxiety and other adverse emotions, so as to avoid disorders of the body’s neurological function. 2, to pay attention to diet, should try to choose more dietary fiber, vitamin-rich food, such as fresh fruits, vegetables, etc.; avoid fatty, fried and other foods with high fat content. 3.Take general antidiarrheal drugs, such as Simethicone, compound phenylephrine, Emmenagogue, etc. under the guidance of doctors. In addition, you can take cholestyramine, a non-activated ion exchange resin, which works by binding bile acids in the intestinal lumen, and generally take 4 to 16 grams orally every day. 4. Chinese herbal medicine can be used to regulate the patient according to his actual condition. This kind of diarrhea mostly appears within 3 to 6 months after surgery, and the symptoms of diarrhea will improve significantly in most patients after a period of conditioning. This may be due to the fact that the common bile duct will gradually compensate for expansion after gallbladder removal, replacing part of the function of the gallbladder.
