How to treat gout in Chinese medicine

  Gout is a disease caused by long-term disorders of purine metabolism, mainly characterized by increased blood uric acid, which mainly manifests as recurrent acute arthritis, chronic arthritis, and uric acid stone deposits, which can lead to joint deformity and mobility disorders in severe cases, and often involves the kidneys, resulting in interstitial nephritis and uric acid kidney stones. It causes great pain to patients and seriously affects their quality of life, so it is important to actively treat gout. Gout belongs to the category of “paralysis” in Chinese medicine, and is also known as “calendar joints” and “white tiger calendar joints”. The etiology of gout is complex, and it is unanimously believed that the disorder of liver, spleen and kidney is the root of the disease, while dampness, heat, phlegm and stasis blocking the meridians and joints are the symptoms of the disease. The treatment of gout is divided into the acute and remission phases. In the acute stage, the symptoms are usually redness, swelling, fever, pain and limitation of movement in the affected joints, and the basic mechanism of the disease is the interconnection of dampness, heat, phlegm and stasis. In the remission stage, the main treatment is to cure the root of the disease, mostly from regulating the liver, spleen and kidney, and the treatment is to warm the spleen and kidney and nourish the liver and kidney.  In the acute stage, there are four symptoms: damp-heat, cold-damp paralysis, stasis-heat and phlegm obstruction: 1. It is accompanied by fever and thirst, restlessness, and yellow urine. The tongue is red, the coating is yellow and greasy, and the pulse is slippery. Treatment is to clear heat and remove dampness, ventilate the ligaments and relieve pain. The formula uses Si Miao San plus and minus, commonly used drugs: Atractylodes macrocephala, Phellodendron chrysanthemum, Chuan Niu Knee, Coix seed, Tu Fu Ling, Dioscorea, Gentiana macrophylla, Slippery Rock, Zeligia, Che Qian Zi, Lonicera, Zhi Mu, Sheng Shi Fa, Wei Ling Xian, Papaya, etc.  2, cold damp paralysis type: most of the joint pain and swelling, localized cold, cold pain aggravated, with warmth pain reduced, joint movement is limited, accompanied by fear of cold body cold, heavy limbs, urine clear and long, loose stool, light tongue, white greasy moss or thin white, tight or sunken tight pulse. Treatment is to warm the menstruation and disperse cold, remove dampness and relieve pain. The formula uses Gui Zhi Wu Tou Tang with addition and subtraction. Commonly used drugs: Chuan Wu, Gui Zhi, Atractylodes, Fu Ling, Ze Xie, Atractylodes, Coix Seed, Ephedra, Hossein, Dou Wu, Fang Feng, Dioscorea, Fang Ji, Papaya, Qiang Wu, etc.  3. Stasis and heat interconnection type: Mostly red, swollen and tingling joints, local swelling and deformation, obstruction of movement, purple and dull skin color, slightly hard when pressed, with hard nodes around the lesion. The tongue is dark or with petechiae, the coating is thin and yellow, and the pulse is thin and astringent or sunken. Treatment is to activate blood circulation and resolve blood stasis, clear heat and relieve pain. The formula is based on the addition and subtraction of Body Pain and Stasis Soup. Commonly used drugs: Gentiana macrophylla, Chuanxiong rhizome, Tao Ren, Hong Hua, Qiang Wu, Myrrh, Angelica sinensis, Wu Ling Li, Mulberry branch, Sheng Di, Dan Pi, Lonicera japonica, Huai Niu Knee, Dan Shen, Radix Paeoniae, Di Long, Stiphulae, Wu Zhi Snake, etc.  4.Phlegm obstruction type: Mostly joint swelling, or even diffuse swelling around the joint, or limb soreness and numbness pain, or see lumps of hard nodes without redness. Accompanied by dizziness, floating face and feet swelling, chest tightness and abdominal gangrene. The tongue is fat and dull, with white greasy coating and a sunken or slippery pulse. The treatment is to strengthen the spleen, resolve phlegm, dispel dampness and promote circulation. The formula is based on Coix Seed Soup with Addition and Subtraction, commonly used drugs: Coix Seed, Atractylodes Macrocephala, Atractylodes Macrocephala, Chen Pi, Citrus Aurantium, Sempervis, Houpai, Bacopa Monnieri, Zhuyu, Zhe Bei, Cyperus rotundus, White Mustard Seed, Dou Shu, Fang Feng, Qiang Wu, Ze Di, Fang Ji, Tu Fu Ling, Dioscorea, etc.  In remission, there are two types of symptoms: spleen and kidney yang deficiency and liver and kidney yin deficiency: 1. It is accompanied by shortness of breath and weakness, coldness in the waist and knees, frequent nocturnal urination, long and clear urine, abdominal distension and loose stools. The tongue is light and fat, with thin white coating, and the pulse is sluggish or sunken. Treatment is to warm the spleen and kidney, dispel dampness and resolve turbidity. The formula is based on the addition and subtraction of Radix Rehmanniae, commonly used drugs: Radix Rehmanniae, Cinnamon, Rhizoma Atractylodis Macrocephalae, Ginger, Eucommia, Epimedium, Bonesetter, Cistanches, Bonesetter, Bupleurum, Poria, Semen Coix, Rhizoma Dioscorea, Atractylodes Macrocephalae, Atractylodes Macrocephalae, Pericarpium Citri Reticulatae, Phellodendron, and Radix et al.  2, liver and kidney yin deficiency type: see joint pain, recurrent attacks, long lasting, or see local joint deformation, visible nodules, skin numbness, flexion and extension unfavorable. With soreness and weakness of the waist and knees, dizziness and tinnitus, red cheek and dry mouth. The tongue is dry and red with little coating, and the pulse is thin or fine. Treatment is to tonify the liver and benefit the kidney, dispel wind and eliminate dampness. The formula is based on Duluxiaosheng Tang with addition and subtraction. Commonly used drugs: Duluxiaosheng, Mulberry, Eucommia, Niubizi, Radix et Rhizoma, Fructus Lycii, Rhizoma Dioscoreae, Yam, Cornu Cervi Pantotrichum, Poria, Huai Niu Knee, Radix et Rhizoma Ligustrum, Tortoise Board, Turtle Nail, Tu Fu Ling, Dioscorea, Zedoary, Coix Seed, Gentiana, etc.  In the treatment of this disease, attention should be paid to the difference between the acute phase and the remission phase. Attention should be paid to the regulation of diet, more water should be drunk, less meat and more vegetables, and purine-rich foods such as animal offal, meat soup, fish, shellfish and beans, as well as alcohol should be prohibited. Moderate physical exercise should be performed during the remission period.
