Does an enlarged heart always result in symptoms?

Not necessarily.
If the enlarged left ventricle due to early hypertension is myocardial compensation, there can usually be no obvious clinical symptoms at this time, except for the primary cause or structural abnormalities of the heart itself. However, if the structural abnormality or haemodynamic disturbance is not corrected in time, severe clinical symptoms, such as dyspnoea and chest tightness, can occur when the lesion exceeds what the heart can compensate for.
