The summer heat has arrived, beer, seafood and various kinds of marinated dishes have become essential delicacies on people’s table, and while people enjoy them, a significant number of people are suffering from gout. Since the beginning of summer, the number of gout consultations in the Department of Rheumatology and Immunology has increased dramatically. In June, three patients with gout were admitted to the rheumatology ward, all of whom had serious kidney damage, including a 74-year-old female patient with renal failure, whose creatinine had reached 440umol/l. 440umol/l, and the patient’s gout attack time was only 4 years, and she did not go to the rheumatology department for systematic and standardized treatment during the 4 years, but only took some oral pain medication and antibiotics without any therapeutic significance during the attack, which was very distressing. The actual gout is a stubborn disease that not only brings harm to the body, but also has a great impact on the psychology of the patient, so what are the dangers of gout? If there is no systematic treatment for gout, and the uric acid in the body is not reduced to a reasonable level in time, then the uric acid may be crystallized in the kidney parts of the body, resulting in kidney function can not work properly, and in serious cases, kidney failure. Research has found that if gout is not properly treated, persistent hyperuricemia will cause excessive uric acid crystals to be deposited in the coronary arteries, which, together with the hyperagglutination of platelets, will accelerate the progress of atherosclerosis and cause serious consequences, leading to myocardial infarction, which can be life-threatening. Gouty kidney stones According to statistics, the chance of kidney stones in gout patients is about one thousand times higher than that of normal people; the more uric acid in the urine and the more acidic the pH, the more likely stones will occur. 4. Hyperlipidemia People with gout often overeat and are obese, so they have a lot of combined hyperlipidemia, which is closely related to the occurrence of atherosclerosis. If the oral glucose load test is done for gout patients, it is found that 30-40% of them have “mild non-insulin-dependent” diabetes mellitus; that is caused by low insulin sensitivity due to obesity and overeating, if diet therapy can be used early and weight control, insulin sensitivity can be recovered soon. Gout patients about half of the combined hypertension, in addition to the above renal dysfunction caused by renal hypertension, gout patients combined with obesity is also one of the reasons. Gout is not only harmful to our body and mind, so gout patients should go to the hospital as soon as possible for treatment, delay will only aggravate the disease, which is not conducive to the treatment of gout. At present, the following problems exist in the treatment of gout: 1. emphasis on pain relief and neglect of gout prevention; 2. focus on the improvement of local joint symptoms and neglect of gout-related complications; 3. lack of staging, grading and comprehensive and combined treatment concepts; 4. lack of rationality in the use of colchicine, lack of rigor in the use of glucocorticoids, and lack of specificity and flexibility in the use of NSAIDs; 5. lack of targeting and flexibility in the use of glucocorticoids; and 6. 5. single treatment means, poor treatment drugs, and lack of strategy and advancement in the overall treatment plan. In conclusion, the treatment of gout is not just analgesic treatment, but should also include uric acid-lowering treatment, treatment to improve local joint signs, treatment to prevent gout-related complications and treatment to prevent gout recurrence. In other words, the treatment of gout is not a single treatment but a comprehensive treatment. Comprehensive treatment is intertwined, because gout can involve all parts of the body except the central nervous system, and neglecting any of the above treatments can cause serious damage to the organism.
