Gout is a group of diseases caused by disorders of purine metabolism, mainly in men, and is characterized clinically by hyperuricemia and the resulting recurrent episodes of gouty acute arthritis, gouty stone deposits, gouty chronic arthritis and joint deformities, often involving the kidneys, causing chronic interstitial nephritis and uric acid kidney stone formation, etc. The primary gout is most often acute gouty arthritis, with an acute onset. It is a condition that occurs in the joints of the lower extremities and can reach its peak within a few hours, with obvious heat and pain in the joints and surrounding soft tissues, and is more common in the middle of the night, lasting several days to several weeks in complete remission, but can recur. It is the main target of acupuncture treatment. This disease in China in the past for rare disease, in recent years due to the improvement of nutritional conditions, the average life expectancy increased, there is a tendency to increase the number of cases. Acupuncture treatment for this disease was first seen in 1981. However, it was not reported until 1991, when a large number of clinical articles appeared in the press. In terms of treatment methods, besides body acupuncture, there are also plum blossom acupuncture or trigeminal acupuncture, fire acupuncture, cupping, intradermal acupuncture and auricular acupuncture, etc., and there are also treatments with tui na or internal and external Chinese medicine. The treatment targets are mainly patients with acute gouty arthritis, but also those with gouty stone deposits. The total effective rate is over 90%. Many patients have not seen recurrence after more than one year of follow-up, which indicates that there is a good long-term effect, so acupuncture treatment of this disease has a certain promotion value.
Treatment
Body acupuncture
I. Acupuncture points
Main acupuncture points: in 2 groups.
1. Foot San Li, Yang Ling Quan and San Yin Jiao.
2, Quchi.
Supporting points: divided into 2 groups.
1, medial ankle: Taixi, Tai Bai, Dadun; lateral ankle: Kunlun, Qiu Hui, Foot Lin Weeping, Shuang Bone.
2. Hegu.
II. Treatment
Take the first group for each lesion in the lower extremity and the second group for each lesion in the upper extremity. The main points are the main points, and the matching points are added according to the location. The needle is inserted with a 1-1.5-inch No. 28 milli-needle, and after obtaining the Qi, the twisting and lifting technique is used to supplement the diarrhea; the diarrhea method is used in the acute phase, and the flat supplement and flat diarrhea method is used in the recovery phase, and the needles are retained for 30 minutes. Efficacy criteria: clinically cured: symptoms and signs disappear, blood uric acid drops to normal, no recurrence within 1~1.5 years; effective: symptoms and signs basically disappear, blood uric acid drops, the interval between attacks is significantly prolonged; ineffective: symptoms, signs and blood uric acid examination are not improved.
The above method was used to treat patients with gouty arthritis. According to the above criteria, 70 cases were clinically cured, 9 cases were effective and 3 cases were ineffective, with a total effective rate of 92.3%.
Blood stabbing
I. Acupuncture points
Main acupoints: in 2 groups.
1. aye-point, tai-chong, nei-ting, and corresponding points.
2, Quchi, Yangchi, Yangxi, Taichong, Qiuhui, Taixi, Yanglingquan, and Blood Sea.
Location of the A-Yi point: the most obvious place of redness, swelling and heat pain.
Corresponding point location: the corresponding part of the A-Yi point on the healthy side of the hand.
II. Treatment
One group is taken at a time, and the two groups can be applied alternately or as a single group. The first group is taken each time, only the affected side of the point; the second group is taken each time 2 to 3 points, alternately selected, which, in addition to Yangchi, Taixi, Blood Sea to take the affected side, the rest are taken bilaterally. The first group of acupuncture points: first puncture the A-Ye point with a trigeminal needle, bleed a few drops, and then use a No. 26 1.5-inch milli-needle to puncture the corresponding point 1 needle, the affected side of Taichong, Nei Ting and three needles at an angle of 15 degrees around the A-Ye point (the tip of these three needles point to the trigeminal needle bleeding), using the diarrhea method, and retain the needle for 30 minutes. The second group of stabbing method: in the selected acupuncture point area first pat with the fingers several times to make local congestion, perform routine sterilization, bet on the hand to press both sides of the acupuncture point to make the skin tense, with a small trigeminal needle, quickly puncture the acupuncture point, the depth depends on the acupuncture point. Squeeze bleeding, add fire cupping in some points, bleeding amount to 3-10ml is appropriate. Disinfect the area and add a dressing to fix it. The above two methods of treatment 1 to 2 times a week, 3 to 7 times a course of treatment, the course of treatment interval of one week.
Evaluation of therapeutic effect
A total of 53 patients with gouty arthritis were treated, and 31 cases were clinically cured and 22 cases were effective according to the aforementioned criteria, with an overall efficiency of 100% [5,6].
Fire acupuncture method
I. Acupuncture points
Main acupoints: xingcheng, taichong, nei ting, and shigu.
Supporting points: Qiu Hui, Du Du, Tai Bai, Blood Sea, Di Yu, Feng Long, Spleen Yu, Tai Xi, and San Yin Jiao.
II. Treatment
Take 2 main acupoints at a time, and 1 to 2 matching acupoints according to the condition. Use coarse fire needles for the foot acupoints and fine fire needles for the acupoints above the ankle joint. Needle foot acupuncture points, so that the patient to take an upright position or sitting position, feet hanging down, under the foot pad several layers of grass paper, acupuncture points line iodine, alcohol after strict disinfection, the fire needle in the alcohol lamp burned until red to white, the point is aimed at the quick stabbing out, the depth of 0.3 ~ 1 inch or so, each point stab 1 ~ 3 needles, after the needle that there is dark red blood from the needle hole, until the bleeding up to 10 ~ 30ml before the bleeding can be stopped. In general, the bleeding is initially dark red, and will stop on its own when the blood is dark to light, if the bleeding does not stop, pressure can be applied to stop the bleeding. The points above the ankle should be taken in a sitting position, and each point should be pricked with 1 needle. In acute cases of gouty arthritis, several stitches can be applied to the red and swollen affected area to drain the pulpy exudate. The above method is used once a week, and the patient is instructed to keep the needle holes clean for 48 hours.
Note: This method is prohibited for patients with blood disorders.
Evaluation of efficacy
The above method treated a total of 157 patients, the results of clinical healing 123 cases, effective 25 cases, invalid 9 cases, the total effective rate of 94.3%. It was found that the amount of bleeding from the fire acupuncture point of the foot (up to 30 ml) was the key to a single cure.

Evaluation of therapeutic effect
A total of 39 cases were treated with this method, and the results were clinically cured in 24 cases and effective in 15 cases, with an efficiency of 100% [9].
Body acupuncture plus pointer
I. Acupuncture points
Main acupuncture point: A-Ye point.
Location of the A-Ye point: the area where the gout stones are located. Gout stones are mostly raised nodules, as small as sesame seeds or as large as eggs, and are usually found in the ear, toes, fingers, and elbows.
Treatment
After finding the A-Yi point, stab 4 needles into the base of the gout stone from the left to the right, and then stab 1 needle along the middle of the gout stone in a direction perpendicular to the needle into the base of the gout stone, using the method of lifting and twisting, and retaining the needle for 20 minutes after obtaining the Qi. A total of 10 patients with gout stones were treated with this method, and all of them were basically cured [10].
Comprehensive method
I. Acupuncture points
Main acupoints: ayurveda, san yin jiao, qiu xu, tai bai, tai chong, and nei ting.
Supporting acupoints: Da Du on the toe, Shang Qiu on the ankle, and Calvaria on the knee.
Location of the A-Yi point: at the redness and swelling.
II. Treatment
The main acupuncture point is the main point, and 3-4 points are taken at a time, with additional matching points according to the site of the disease. For other points, use a No. 28 1.5-inch milli-needle. After acquiring qi, apply the lifting and twisting technique or use the diarrhea method in the acute phase, and the flat patching and flat diarrhea method in the recovery phase. Retain the needle for 30 minutes, and apply the technique once every 10 minutes. Once a day.
Add the following herbs: 3g of Fangqi, 12g of Astragalus membranaceus, 12g of Atractylodes Macrocephala, 15g of Morus alba, 30g of Lonicera japonica, 12g of Radix Achyranthes bidentata, 18g of Papaya, 12g of Dilong, 15g of Paeonia lactiflora, 18g of Sangchuang, 4 whole scorpions, 2 centipedes. The latter two are dried and finely powdered and taken in 2 doses. The above medicine is taken as 1 dose per day, divided into 2 doses with water decoction. Third, the efficacy evaluation
The above method treated a total of 69 cases (10 of which were treated with acupuncture plus external application of Chinese medicine), the results: 34 cases were clinically cured, 29 cases were effective, and 1 case was invalid, with a total efficiency of 98.6%.

As the abnormal increase of uric acid in the body is related to the decrease of kidney excretory function, patients with gout disease in the resting stage should tonify kidney qi and increase excretory function. In clinical practice, acupuncture points such as Taixi, Fuyao, Shenmen, Quchi, Hegu, Feosanli, Guanyuan, Qihai, and Doudao can be selected and tonicized by milli-needle twice a week, 10 times a course of treatment.
