Current status and progress in the diagnosis and treatment of varicose spermatocele

  Varicocele occurs in 15% to 20% of adolescent or adult males. Many patients present to urology clinics, about 40%.  Varicocele is present in infertile men.  A comprehensive analysis of information provided by the World Health Organization shows that treatment of patients with varicocele has no significant impact on male fertility …… 1/3 of adolescent boys with second degree varicocele and more than half of those with third degree varicocele have decreased testicular volume on the affected side, with 8O% of these boys having a significant increase in testicular volume after surgical correction. There is a significant increase in volume, and about 67% of them have improved sperm parameters after varicocele correction. There are 2 indications for surgical correction of varicocele: pain and infertility.  There are many surgical approaches, including the retroperitoneal route, the inguinal route and laparoscopic surgery, but it is difficult to separate the testicular arteries from the spermatic veins and lymphatic vessels in all of these conventional procedures, resulting in early postoperative epididymitis, scrotal edema and, in the most serious cases, testicular atrophy due to ligation of the testicular arteries and testicular atrophy due to omission of the veins.  Currently, the international standard surgical method is microscopic spermatic vein ligation through the external inguinal ring, which can clearly distinguish the testicular artery, spermatic vein and lymphatic vessels under the microscope, and precisely preserve the testicular artery and lymphatic vessels. At present, not many hospitals and departments in mainland cities perform this procedure. I have been performing this surgery in my department for more than a year and have accumulated some experience. I have received guidance from Shanghai Renji urological male specialists, Professors Li Zheng and Sun Kai, and have successfully completed more than 60 cases with precise surgical efficacy and few complications.
