Varicocele is a common disease in young and middle-aged men, which refers to the dilatation, tortuosity and elongation of the vascular plexus of the spermatic cord (vascular plexus) due to stagnation of blood flow in the spermatic cord. Varicocele can be associated with testicular atrophy and impaired sperm production, which in turn can be a major cause of male infertility.
The hazards of varicocele are as follows.
1, varicose spermatozoa can sometimes affect fertility. Varicocele can sometimes affect fertility. 9% of people with varicocele are infertile and 39% of men with infertility are caused by varicocele. Severe cases can cause testicular atrophy. The reason is that the temperature in the affected scrotum increases and reflects to the opposite side, causing the spermatogonia to degenerate, atrophy and reduce the number of sperm; or because the pentraxin or steroid secreted by the left adrenal gland flows back into the testis through the left spermatic vein, causing a reduction in the number of sperm.
The scrotum or testicles on the affected side have a feeling of swelling or pain, the scrotum is enlarged, and the scrotum and testicles on the affected side are lower than those on the healthy side when standing up. The scrotal surface can be seen as dilated and tortuous veins. There are earthworm-like soft masses when touched, and the symptoms can be reduced or disappeared by lying down.
Patients may have symptoms of neurasthenia, such as headache, fatigue, hypersensitivity, etc. Some patients have sexual dysfunction. Some patients have sexual dysfunction.
4.Patients can be completely asymptomatic, only found during physical examination. Varicocele is not a minor problem, and it is a danger to men’s physical and mental health that cannot be ignored. In addition to causing male discomfort, it can also have a great impact on fertility. The specific hazards are as follows.
1) Affects male fertility. According to medical statistics, about two-thirds of men suffering from varicocele have abnormal semen, which can lead to a decrease in the number of sperm, low motility and an increase in deformed sperm, thus leading to infertility.
2) Causes local discomfort. It is manifested as cramping and pulling pain at the scrotum, which is significant when standing and moving and can be relieved by lying in bed. It can also be accompanied by neurological weakness due to the patient’s worry and anxiety, causing mood disorders, fatigue, insomnia, etc.
3) Causes sexual dysfunction. Some patients with varicocele, if left untreated for a long time, may develop sexual dysfunction such as low libido, decreased sexual pleasure, painful intercourse, erectile dysfunction and premature ejaculation.
In the past, vein ligation was performed in the scrotum in earlier years because of the emphasis on the thickened varicose vessels of the trapezius plexus. The recurrence rate proved to be high. It is now thought to be caused by backpressure of the venous blood column.
The disease is not treated exclusively with surgery, which, after all, has a high recurrence rate. After the ligation of the internal spermatic vein, the venous blood returning from the testes and parametrium may be returned to the inferior vena cava along the external spermatic vein. Or back to the femoral vein through the subcutaneous venous branches below the external ring.
1.Surgery.
In severe varicose veins with valve dysfunction, the doctor can surgically draw the problematic veins out of the body, but this treatment option is limited to superficial varicose veins.
If the problem comes from perforated veins, the doctor will use an endoscope to bundle up the fascia of the perforated veins to prevent blood from entering or leaving the perforated veins, and the problem will be solved, but it is easy to recur.
2.Medication.
Mild varicose veins, though painless and harmless, are an eyesore. Summer is coming, when wearing shorts or swimsuit, feet swell up with curved, blue and purple “veins”, it is simply a big disgrace.
3.Pressure therapy.
Deep varicose veins, can use elastic bandages wrapped lower limbs, pressure on the veins to stop local hypertension, promote blood flow.
4.Sclerotherapy.
A sclerosing solution is injected into the varicose part of the vein to make the vein shrink and narrow, limited to treating varicose veins below the knee.
Prevention.
1, abstain from intercourse, often wear tight underwear or use scrotal rest to prevent scrotal drop.
2, pay attention to keep a happy mood, avoid anger injury liver, pay attention to diet adjustment, avoid eating spicy food.
3, pay attention to the combination of work and rest, to prevent strenuous exercise, heavy physical labor and long standing.
(1) wear underwear, should be made of cotton material with high elasticity, should not wear tight underwear, in order to facilitate heat dissipation and sweat absorption, reduce the local temperature of the scrotum and reduce the effect of gravity.
(2) If there is discomfort of scrotal swelling and obvious abnormality, you should go to the hospital for treatment in time. Regarding the treatment of varicocele, those who are asymptomatic or have mild symptoms can wear elastic underwear or use scrotal brace. For infertility with more severe symptoms or with sperm abnormalities, high level ligation of the internal spermatic vein is feasible.
(3) Do a good job of regular physiological health education and give them the knowledge about varicocele to eliminate their psychological burden.
(4) Reasonable schedule training, so that the combination of work and rest, to avoid excessive prolonged weight-bearing standing.
Generally speaking, 80% of patients who undergo surgery can recover their fertility, but this does not exclude other diseases as influencing factors, so different treatments should be done for different causes.
So about the left spermatic varicocele surgery can be fertile depends on the post-operative results and recovery status and other diseases, but patients do not have to worry too much, as long as actively identify the cause of the symptomatic treatment, natural fertility will be restored.
