Basic elements of chiropractic and precautions

【Overview】 Chiropractic, also known as “spinal dwelling (fixed point) rotational reset method”, is a method of treating spinal injuries and other diseases by dividing tendons and plucking, pressing and sparing the back of the spine and other rectification techniques to promote the smooth flow of Qi and blood in the veins, so that the sick vertebrae return to normal. This therapy has been applied by medical practitioners for a long time. In the Qing dynasty, “The Golden Guide to Medicine? Orthopedic Heart Method Essentials” said: “The spine …… first affected by wind and cold, after being bruised and injured, stasis agglutination. If the spine tendons Long up, the bone seam must be wrong, it becomes hunchbacked shape. When first rubbing tendons, so that it and soft; then press its bones, slowly close the seam, the back brawn began to straighten.” The etiology, clinical manifestations, and rehabilitation techniques of injured spinal lesions have been clearly described. In recent times, the treatment scope of this therapy has developed a lot, not only for the cervical spine, lumbar spine spine deviation and other orthopedic diseases, but also can be widely used in certain diseases caused by spinal lesions. [Basic content] Through the treatment of spinal (fixed-point) rotational repositioning manipulation, the affected vertebral space and the fibrous ring and intervertebral ligament can be induced to rotate and pull, thus generating peripheral pressure on the herniated nucleus pulposus and making the herniated object easy to retract; by correcting the deviated spinous process, the vertebral joint can restore the normal (or compensatory) anatomical position and make it fit with the surrounding muscle groups (i.e. what ancient medical texts call “bone in the suture” and “tendon in the groove”), relieving the compression of the nerve roots by the joint capsule and the yellow ligament, and improving the blood flow of the vertebral artery. In addition, rotational manipulation applied to combined small joint stiffness and coagulation can also loosen adhesions, increase range of motion, and relieve pain. When applying this therapy, the operator should first touch the patient’s spine with his fingers to check whether the position of the spinous processes of the relevant vertebrae is normal, whether there is pressure pain next to the affected spine, and whether the paravertebral tendons are thickened, contracted, or peeled, etc., and then apply the corresponding corrective techniques for treatment. For example, in the examination and diagnosis of disc herniation, Feng Tianyou proposes the following four features: (1) the spinal process of the affected vertebra is in a skewed position. When the physician palpates the spine with the thumb, a series of signs of the distorted spinous processes can be detected; (2) the gap between the upper and lower spinous processes of the affected vertebrae is wide and narrow; (3) the affected vertebrae have pressure pain next to the spinous processes or are accompanied by radiating pain to the lower extremities; (4) the affected supraspinal ligaments have striations, are dull and thick to the touch, and the pressure pain is obvious. Anyone who has one or two of these clinical features can confirm the diagnosis. Indications】 This treatment has good effect on injury spinal lesions, such as cervical spondylosis, lumbar disc herniation and certain injury paraplegia. Some patients can even receive immediate results. In addition, hypertension, cardiac arrhythmia, post-traumatic brain syndrome, weakened vision or blindness, deafness and other diseases caused by spinal disease can also be achieved in the process of rectification. For cervical spondylosis, post-traumatic dizziness, post-traumatic brain syndrome, deafness of the ears and eyes, and shoulder and arm pain and carbuncle numbness, which are mainly symptoms of the head, face, neck and arm, the diseased vertebral areas should be examined and determined in the cervical vertebral segment and the corresponding restorative techniques should be applied. For cardiac arrhythmia, gastroparesis, intercostal neuralgia, diarrhea and other symptoms mainly in the chest and abdomen, the thoracic vertebrae should be examined and the diseased vertebrae should be identified, and the corresponding manipulation should be applied. For patients with lumbar pain, lower limb pain and numbness, and urinary and fecal disorders, the examination and rectification techniques should focus on the lumbar vertebrae. Contraindications】 Patients with acute infectious diseases or serious organic diseases such as heart, lung, liver and kidney, tumor and bone tuberculosis, etc., even if the operator is extremely skillful, are also cautious to use this therapy to rectify the manipulation. Precautions] 1, the application of this therapy, accurate positioning of the diseased vertebrae is the premise of the effect, skilled rehabilitation is the key to improve the effectiveness of the technique. Check the inaccurate positioning or omission of diseased vertebrae, skewed spine direction error, can make the curative effect is not obvious, and even aggravate the disease. Rectification techniques must be accurate, gentle force, do not violent. 2, the treatment of a rectification can not set the crooked spine, should not be continuous treatment, can be combined with the tendons, take point massage massage and other massage techniques to release the spasm, and then apply the rectification techniques. Some patients need to be treated once in several days, and 4 or 5 times in a row to correct the distorted spinous process, so do not be in a hurry. 3, in the cervical spine area when applying this therapy to rectify, improper manipulation may stimulate the vertebral artery and produce deficiency, individual patients may cause medical spinal injury and lead to serious consequences such as high paraplegia. Clinical data show that patients with cervical spine syndrome, lumbar disc herniation and other diseases can be relieved or show effects by applying this therapy four or five times on average; some cases can be healed in one treatment. Since the clinical report of several cases of high paraplegia in patients with cervical spondylosis using this therapy, some people think that it is inappropriate to use the rectification techniques of this therapy in view of the anatomical structure of the neck. However, the majority opinion is that cervical spondylosis is a syndrome, and this therapy is a targeted treatment for the neck symptoms caused by interlocked and disturbed small joints and distorted spinous processes. Generally speaking, as long as the indications and the rules of therapeutic techniques are strictly mastered, it is extremely rare for even beginners to cause medically induced injuries to patients when applying this therapy. In addition, the principle of appropriate rest and functional exercise after manipulative treatment is also essential to consolidate the effects of treatment. It is worth mentioning that the role of chiropractic in the treatment of spinal disorders has been clinically proven and recognized by scholars and clinicians, but its role in the treatment of so-called spinal-related disorders is not widely recognized and needs to be further studied. Although chiropractic has significant clinical utility in some conditions, many clinicians are discouraged by the potential dangers. However, the vast majority of scholars believe that it is generally safe as long as the indications are strictly grasped, the patient is carefully examined, and the practice is performed with a solid knowledge and skillful and careful manipulation. This, on the other hand, also requires patients to go to a regular hospital for treatment whenever possible to avoid danger. This is because safety is always the first priority for doctors and patients.
