Hemangioma is caused by the abnormal proliferation of vascular endothelial cells, it is one of the more common skin tumors in clinic, especially the fast-growing hemangiomas in children and hemangiomas on the face of adults, which can cause serious facial deformities and dysfunctions if not treated in time. Pingyangmycin is a new type of domestic anti-tumor antibiotic, which is a variety of bleomycin A group (A5) produced by Pingyang streptomycin, and it is a new type of anticancer drug that has little effect on the immune function of the body and no obvious damage to the hematopoietic function, and it has strong anti-tumor activity and low toxicity. The principle of its action is that after entering into the tumor body, the drug specifically combines with intracellular DNA, prompts the oxidation of divalent iron into trivalent iron, generates free radicals, then acts on DNA, inhibits the synthesis of cellular DNA and cuts off the DNA strand to interfere with cell division and proliferation, and inhibits the growth of vascular endothelial cells. Therefore, injection of Pingyangmycin into hemangioma can rapidly inhibit the proliferation of vascular endothelial cells and promote the regression of hemangioma without scarring of the skin. In this paper, the methods, efficacy, care and adverse reactions of the treatment of hemangioma by injection of paxillin are summarized. 1.Treatment method Before the injection of pingyangmycin, all the patients should have routine blood test and chest X-ray if necessary. Take 4~8mg of pingyangmycin, dilute it with saline or 1% lidocaine injection 2~4ml, the amount of drug and dilution concentration is related to the site of the lesion, the size of the volume, the type of the lesion and the age of the patient[1]. Generally injected according to 1~2ml dilution/cm2 tumor area, superficial hemangioma with less drug, deep (such as cavernous, mixed) hemangioma and larger children with appropriate increase in the amount of drug, do not give too large a dose at one time, to avoid hemangioma and local tissue necrosis. After injection, in order to avoid the rapid entry of drugs into the blood circulation, the central empty ring-shaped gauze roll can be used to compress the lesion around, so that the drug can give full play to its effect in the hemangioma, and the time of half an hour is appropriate [2]. For those who are not cured by one injection, the injection can be repeated at an interval of 7~15 days, and after the local tissue swelling is eliminated, a second injection is appropriate, generally 3~5 times a course of treatment, and the total amount of the drug used should not be more than 64mg at most. The method of injection of pingyangmycin can be different due to the different parts of the hemangioma. Generally, 0.5~0.8ml intradermal injection and whitening of the surface of the tumor are enough. Be careful to prevent the extravasation of drug solution to stimulate the peripheral skin tissues. The perineal tumors have loose local tissues, easy to be contaminated, and should be injected centrally and diversely. Pay attention to local cleanliness. For cystic tumors, the fluid in the cystic cavity should be withdrawn first, and then inject the drug slowly to ensure that the drug is injected into the cavity. Depending on the color of the liquid during puncture, the tumor can be initially classified and characterized. Large and deep tumors should be staged and injected with both shades of liquid [3]. 2. Nursing Psychological care before injection treatment:Pediatric hemangioma is a common benign tumor, early growth rate is faster, making parents worried, mental and psychological pressure. Parents should make patient and careful explanation to relieve all kinds of worries and pressure, and actively cooperate with the treatment to achieve the expected results. Local skin care before injection treatment: the local skin of the tumor must be free of infected lesions. If the skin surface is ulcerated, vesicles, oozing, blisters and other phenomena should be controlled after infection for local injection treatment[3]. 3.Efficacy Literature reports after clinical practice, that pingyangmycin injection on simple cavernous hemangioma effect is the most ideal, no sequelae, the efficiency basically reaches more than 90%; and for simple and mixed skin manifestations, should pay attention to the concentration of the drug, the method of operation, post-injection care, etc., otherwise it is easy to occur epidermal ulceration, delayed healing, scarring, etc., with the pingyangmycin local injection combined with laser treatment, can achieve the ideal results. The local injection of pingyangmycin combined with laser treatment can achieve ideal results [4]. 4, local injection of pingyangmycin its adverse reactions are mainly fever, metamorphosis, pulmonary toxic reactions and skin reactions, etc. [6]. 4.1 Fever As a result of stimulating the release of endogenous pyrogen. Its incidence is reported to be 44% in the literature, but the probability of fever with topical administration is low, with an incidence of 10% or less. Hormone injection can inhibit the release of endogenous pyrogen, control the febrile reaction, and also prevent the allergic reaction. Therefore, hormone injections can be given before or after paxillin injection to prevent febrile reactions. 4.2 Anaphylaxis may occur after the first dose, or after several doses, during the administration, or several hours to 4 hours after the administration. It is recommended to do the drug sensitivity test according to the instructions before use, pingyangmycin 1mg, physiological saline 1ml, 2% lidocaine 1ml, subcutaneous injection, observe no allergic reaction for 1 hour, and then inject the full amount of body. At least one skin scratch test should be done before local injection. Because some children under 1 year old have dyspnea and stridor, prepare oxygen and sputum suction device when injecting. 4.3 Lung toxicity Acute reaction is manifested as pulmonary edema, chronic reaction is manifested as interstitial fibrosis. Generally, pulmonary lesions appear when the cumulative dosage reaches more than 300mg. The injection volume of hemangioma is far less than 300mg, so the chance of pulmonary toxicity is also very small. No patients with pulmonary fibrosis have been reported in the literature after two years of follow-up. 4.4 Skin reactions Rash may occur, but may subside spontaneously after discontinuation of the drug. Localized erythema and blisters may be treated as surgical wounds. In conclusion, local injection of pingyangmycin for the treatment of hemangioma has the advantages of simple operation, rapid and precise therapeutic effect, small toxic side effects, few complications, outpatient treatment, and not easy to leave deformity, etc. It is especially suitable for pediatric proliferative stage, but it is not suitable for the treatment of hemangioma. It is especially suitable for the treatment of pediatric proliferative hemangiomas, and can also be used as a new way for the treatment of adult facial hemangiomas.
