Ultrasound-guided percutaneous hepatic puncture alcohol injection therapy (PEIT) for the treatment of hepatocellular carcinoma began in the 1980s. The basic principle of PEIT is to use the dehydrating effect of anhydrous alcohol (concentration of 99% or more) on tissues and the coagulation effect on proteins. The purpose is to inactivate cancer cells, inhibit the growth of tumor and prevent its spread. Alcohol is a clarified thin liquid with almost no viscosity, so it is very easy to be injected through a fine needle, and the coagulation area after injection is mainly limited to the alcohol injection area, which has little effect on the peripheral tissues or the whole body. Since the 1980s, PEIT has been rapidly popularized in various provincial and county hospitals in China, benefiting numerous liver cancer patients and becoming the first choice of palliative treatment for patients with intermediate and advanced liver cancer (i.e. those who have lost the chance of surgery and cannot tolerate chemotherapy). Single or multiple foci (not more than 3 in total) with intrahepatic diameter ≤3cm, diagnosed as primary hepatocellular carcinoma by CT or MRI and without portal vein thrombosis and/or extrahepatic metastasis are the absolute indications for this therapy, and the expected effect of treatment is also the best, which can achieve in situ inactivation and high 5-year survival rate. 1.Relative indications: ① cannot tolerate hepatic artery embolization chemotherapy (TACE) or surgery; ② ineffective or incomplete effect of TACE; ③ cannot successfully perform TACE due to technical difficulty; ④ giant hepatocellular carcinoma. 2. Contraindications: ① platelet count <30×109/L; ② uncontrollable ascites; ③ deep jaundice; ④ significantly prolonged bleeding and clotting time; ⑤ alcohol allergy. This treatment is simple and relatively safe. The more common side effects include transient pain at the puncture site and hypothermia; rarer side effects include bleeding, bile duct occlusion, and low liver function, which rarely occur under the premise of strict control of indications. This treatment program is routinely carried out in the Department of Hepatology of our hospital. For patients with complete treatment (in situ inactivation of tumor is confirmed by re-examination of intensive CT), Chinese herbal medicine is used internally to strengthen the spleen, tonify the kidneys, resolve blood stasis and open the ligaments, so as to support the righteousness and eliminate the evil, and take care of both the symptoms and the root cause, which can play a certain effect in enhancing immune function and preventing tumor recurrence and metastasis.
