Treatment of pediatric constipation

Constipation is a common clinical symptom and sometimes may not be considered a disease. It mainly refers to a decrease in the number of bowel movements, a decrease in the volume of stool, dry stool and straining to pass stool. It must be combined with the nature of the stool, the usual defecation habits and the difficulty in defecation to make a judgment of whether there is constipation. Generally speaking, constipation that has not been solved for three days must be promptly laxative.
Constipation is often manifested as follows: less bowel movement and less frequent bowel movements; difficult and laborious bowel movements; poor bowel movements; dry and hard stools and a sense of unclean bowel movements; constipation accompanied by abdominal pain or abdominal discomfort. Some patients also have insomnia, irritability, dreaminess, depression, anxiety and other mental and psychological disorders.
Constipation can be divided into two categories: organic and functional, in terms of etiology.
  Organic.
Organic lesions of the intestinal canal, rectum, and anus; endocrine or metabolic diseases such as diabetes mellitus, hypothyroidism, and parathyroid disease; colonic neuromuscular lesions such as pseudo-intestinal obstruction, congenital megacolon, and megorectum; and pharmacologic factors such as iron, calcium channel antagonists, diuretics, and antihistamines.
  Functional include.
1, low food intake or food lacking fiber or insufficient water, which reduces the stimulation of colonic motility.
2, interference with normal bowel habits due to emotional stress, fast pace of life, changes in the nature and timing of life, mental factors, etc.
3.Multi-sickness and weakness, too little activity, intestinal spasm leading to defecation difficulties, or due to the redundancy of the colon.
Constipation is a more common symptom, the prevalence of constipation in the population up to 27%, the symptoms vary in severity, but only a small percentage of constipated people will seek medical advice. Most people often do not pay special attention to constipation, thinking that constipation is not a disease and does not need treatment, but in fact constipation is very harmful.
The main treatment is life care. For constipation, especially in some children, the symptoms can be particularly severe after suffering from this disease. And because children have a healthy intestinal tract, it is less harmful to their health if they start with their diet. So the first thing is to take reasonable dietary regimen, such as increasing the dietary fiber content, increasing water intake to enhance the stimulation of the colon, encouraging patients to eat more fiber-rich vegetables, more fruits such as kiwi, bananas, pears and watermelon to increase the volume of stool, and drinking more water and less stimulating beverages such as strong tea and coffee, and also advocating oral honey to play the role of laxative, but some of them are allergic to bee flower I advocate that children over 12 years old should take it orally as appropriate. The second thing is to develop good bowel habits, defecate regularly every day to form a conditioned reflex and establish a good bowel routine. Don’t ignore the urge to defecate, defecate in a timely manner, and make the environment and posture of defecation as convenient as possible, so as not to inhibit the urge to defecate and destroy the defecation habit. The third should be accompanied by increased activity, especially outdoor activities.
Some constipation can be treated with medication
1, volumetric laxatives mainly include soluble fiber (pectin, plantain, oat bran, etc.) and insoluble fiber (plant fiber, lignin, etc.). Volumetric laxatives have a slow onset of action with few side effects and are safe, so they have a better effect on mild constipation, but are not suitable as a rapid laxative treatment for temporary constipation.
2, lubricating laxatives can lubricate the intestinal wall, soften the stool, so that the stool is easy to discharge, easy to use, such as corkage, mineral oil or liquid paraffin.
3, osmotic laxatives commonly used drugs are lactulose, sorbitol, polyethylene glycol 4000, etc.. Suitable for blocked feces or as a temporary treatment measure for chronic constipation, is a better choice for constipation patients with poor efficacy of volumetric light laxatives.
4, stimulant laxatives including anthraquinone-containing botanical laxatives (rhubarb, Frangipani, senna, aloe), phenolphthalein, castor oil, diethylstilbestrol, etc.. Stimulant laxatives should be used only when volumetric laxatives and salt laxatives are ineffective, some of them are stronger and not suitable for long-term use. Long-term application of anthraquinone laxatives can cause colonic black stool disease or laxative colon, causing atrophy of smooth muscle and damage to the intestinal muscular plexus, but aggravate constipation, reversible after discontinuation.
6, pro-dynamic agents Mosapride, Itopride has a pro-gastrointestinal dynamics, Pulucapride can selectively act on the colon, can be selected according to the situation.
Acute constipation, abdominal pain, abdominal distension is obvious, the child will cry and restless, can use colon hydrotherapy or clean enema of the device to assist in treatment. The people like to use the warming soap head inserted into the anus laxative method is also safer, to remind everyone is to use less irritating soap head, the size of a small thumb can be, pay attention to the retention of 5-10 minutes is appropriate, the use of open cork is also this principle.
In fact, in Dr. Wu’s opinion, the acute laxation of constipation is easy, lifting the cause of constipation is the most important. In my clinical work, I found that most children with constipation have varying degrees of partial eating habits, and it is necessary to correct the partial eating in order to completely get rid of constipation. There are two reasons for the partiality.
One is the bad habits developed by parents, who listen to their children completely on everything and burn what they like to eat every day, and do not do anything at all to their children if they do not like it.
The second is the imbalance of vitamins and trace elements such as zinc and lead, this situation will have to go to the hospital for diagnosis and treatment.

  10, single species food
Sesame, almonds and other magnesium-rich foods. Japan’s National Institute of Health and Nutrition and Women’s Nutrition University joint research shows that magnesium intake less people are prone to constipation. Magnesium has a light diarrhea, soften the role of stool, moderate intake helps to pass stool. Foods rich in magnesium include peanuts, brown rice, walnuts, bananas, etc.
Kimchi, fermented soy sauce and other foods rich in plant-based lactic acid bacteria. Lactobacillus has the effect of adjusting the intestinal environment and eliminating constipation. However, there are two types of lactic acid bacteria, animal and plant, and the latter can play a role in preventing constipation. Because the latter is more resistant to the acidic environment than the former, it can reach the intestine and function alive, and the survival rate of the latter in the intestine is 10 times higher than that of the former.
Foods rich in oligosaccharides such as asparagus and onions. Oligosaccharides, also known as oligosaccharides, are a type of carbohydrate with a relatively low degree of polymerization. Studies have found that oligosaccharides can increase the level of bifidobacteria in breast milk, helping to regulate the intestinal environment. Other foods rich in oligosaccharides include garlic, coffee, corn, honey, and various types of soy products.
Foods rich in vitamin C such as green peppers and kiwi. Vitamin C in the intestinal tract is one of the foods of lactic acid bacteria, which can increase the number of probiotics and thus indirectly prevent constipation. Other foods rich in vitamin C include fresh dates, oranges and green leafy vegetables.
Foods rich in oleic acid such as olive oil and sunflower oil. Oleic acid has an emulsifying effect and can soften feces in the intestines and promote bowel movements. Oleic acid-rich foods also include nuts such as sesame, peanuts, almonds and walnuts.
Coarse grains and vegetables and other foods rich in dietary fiber. The former can soften stool, increase the number of intestinal probiotics, adjust the micro-ecological balance in the body; the latter can absorb water in the intestinal tract and swell, stimulate the intestinal wall, accelerate intestinal peristalsis and adsorption of harmful substances, and will be expelled from the body. Foods rich in these two kinds of dietary fiber are a variety of coarse grains and beans, such as brown rice, red beans, green beans, kidney beans, etc., mushroom and algae foods, such as fungus, kelp, mushrooms, etc., and a variety of fruits and vegetables, such as tomatoes, burdock, apples, etc.
