What is the last resort in the treatment of refractory mental illness?

  We all know that mental illness is a chronic and prolonged disease, characterized by one exacerbation per episode. The most common treatment for mental illness is medication, sometimes combined with psychotherapy and physical therapy. For refractory mental illnesses, what else is available when the above treatments are ineffective?
