Dietary therapy for gout

1, maintain the ideal weight Epidemiological survey found that the serum urate level is positively correlated with the degree of obesity, body surface area and body mass index. Clinical observation shows that when the weight of obese patients is reduced, the serum urate level is lowered, urine excretion is reduced and gout attacks are alleviated. Some scholars suggest that the daily purine intake should be below 100-150mg, especially the intake of purine-rich food should be limited. Since protein has a special role in the body, excessive protein intake can also increase endogenous uric acid, so it should also be appropriately limited. 3.Encourage the selection of alkaline food Food containing more sodium, potassium, calcium, magnesium and other elements, oxidation in the body to generate alkaline oxides, such as vegetables, Maling potato, sweet potato, milk, etc., physiologically known as alkaline food. Fruits, such as citrus, are also alkaline foods because they are rich in alkaline potassium after metabolism in the body. Increasing the intake of alkaline food can reduce the acidity of serum and uric acid, and even make the urine alkaline, thus increasing the solubility of uric acid in urine. 4. Ensure sufficient urine volume If the patient’s heart and lung function is normal, the urine volume should be maintained at about 2000ml/d to promote uric acid excretion. Therefore, the total daily fluid intake of patients should be 2500-3000ml, and drinks such as ordinary water, tea, mineral water, soft drinks and fruit juices are appropriate. However, strong tea, coffee, cocoa and other beverages have excitatory effects on the plant nervous system and may cause gout attacks, so they should be avoided. In order to prevent urine concentration at night, it is more appropriate to drink water before bedtime or in the middle of the night. Method 1.Limit total calories Total calories are calculated according to the patient’s ideal body weight at rest, usually not more than 105-126kJ(25-30kcal)/kg per day. clinical experience shows that for adult patients who are more than moderately obese (30%-50% overweight), total calories exceeding 6300kJ per day often cannot make weight loss. The following methods can be used for reference to limit total calories and reduce body weight. (1) Patients who are 30%-50% overweight or above, start with 6300kJ/d of total calories and divide it into three meals. After one month, change it to 5460kJ/d; or reduce calories by 2310-4620kJ/d on the basis of the original diet, with the aim of losing 0.5-1.0kg of body weight per week. (2) Overweight or mildly obese people The total calorie starts with 6300kJ/d and is supplied in three meals; or the caloric energy is reduced by 525-1050kJ/d on the basis of the original diet in order to achieve the purpose of losing .5-1.0kg per month. 2, the distribution of the three major nutrients Under the premise of limiting the total calories, the distribution of the three major nutrients are: high carbohydrate, medium protein and low fat. (1) Carbohydrates Carbohydrates, including vegetables and fruits, should account for 65%-70% of the total calories. This is also in line with the national diet, so that it can reduce fat decomposition to produce ketone bodies, which is conducive to uric acid salt excretion. However, sucrose or beet sugar should be consumed as little as possible. (2) Protein Protein should account for 11%-15% of total calories, usually 0.57-1.0g/kg per day, mainly milk, cheese, skim milk powder and egg protein. Because they are high-quality proteins rich in essential amino acids, which can provide the needs of tissue metabolism constantly updated, and contain very little purine, they have little adverse effect on gout patients. However, yogurt contains more lactic acid, which is not good for gout patients, so it should not be consumed. (3) Fats The rest of the total calories are supplemented by fats, usually 40-50 g/d. Since the oxidation of fats produces about two times more calories than carbohydrates or proteins, they should undoubtedly be limited in order to reduce the patient’s body weight. Precautions 1. Avoid drinking alcohol The main component of alcohol is ethanol, which can induce glycogen isogenesis disorder and lead to the accumulation of lactic acid and ketone bodies in the body. The β-hydroxybutyric acid in lactic acid and ketone bodies can competitively inhibit uric acid excretion. 2.Limit purine individually Limiting the intake of drifting purine content should be treated separately according to the severity of the patient’s condition, the stage of the disease, comorbidities and the application of uric acid-lowering drugs, that is, in accordance with the patient’s individual situation. 3.Pay attention to the cooking method of food A reasonable cooking method can reduce the amount of dextrine contained in food, such as cooking meat first and discarding the soup before cooking. In addition, chili, curry, pepper, pepper, mustard, ginger and other foods and spices can excite the plant nerves and induce an acute attack of gout, and should be avoided as far as possible.
