【Abstract】The sutra formula Bai Hu plus Ginseng Tang with addition and subtraction was used to treat 40 cases of achlorhydria. The results were satisfactory in improving blood glucose and eliminating symptoms. Key words】Anti-thirst – heat in the lung and stomach; pathogenesis; Bai Hu plus Ginseng Tang The pathogenesis of anti-thirst is based on Yin deficiency, while dryness and heat are the symptoms. In 2012, the author used Bai Hu plus Ginseng Tang with reduction to treat the upper and middle extremities of extirpation with satisfactory clinical results. 1. Clinical data 1.1 1.1 General data All cases were from our hospital outpatients and inpatients totaling 80 cases, all of which met WHO and DPN diagnostic criteria. The random blind method was used to divide into treatment group and control group, 40 cases in the treatment group, 21 males and 19 females; the average age was 48.9 years; the duration of diabetes mellitus was from 4 to 7 years. In the control group, there were 40 cases, 19 males and 21 females; the average age was 50.2 years; the duration of diabetes mellitus was 3 to 6 years. The differences between the two groups in terms of gender, age and duration of disease were not statistically significant (P>0.05) and were comparable.1.2 Diagnostic criteria: All cases in this group met the diagnostic criteria proposed by the current international general WHO expert committee on diabetes (1999) as follows: (1) Fasting plasma glucose (FPG): FPG 3.9-6.0 mmol/L (70~ 108mg/dl) is normal, 6.1-6.9mmol/L (110-125mg/dl) is impaired fasting glucose (IFG), ≥7.0mmol/L (126mg/dl) should be considered as diabetes mellitus and needs to be confirmed again on another day. (2) 2-hour plasma glucose (2hPG) in OGTT: 2hPG <7.7mmol/L (139mg/dl) is normal glucose tolerance, 7.8 to <11.1mmol/L (140-199mg/dl) is IGT, ≥11.1mmol/L (200mg/dl) should be considered diabetes mellitus, which needs to be confirmed again on another day. (3) Diagnostic criteria for diabetes mellitus: diabetic symptoms + plasma glucose ≥ 11.1mmol/L (200mg/dl) at any time, or FPG ≥ 7.0mmol/L (126mg/dl), or 2hPG ≥ 11.1mmol/L (200mg/dl) in 0GTT. For those with atypical symptoms, a repeat confirmation is required for the diagnosis to be established. 1.3 Chinese medicine identification criteria Refer to the Guidelines for Clinical Research on New Chinese Medicines [2]? and the “Chinese medicine diagnosis and treatment of internal diseases in Chinese medicine” [3], we chose the evidence of heat in the lung and stomach. It is characterized by: common heart trouble, thirsty and induced to drink, eating a lot and good hunger, dry stool, yellow and dry tongue coating, and large pulse. 2. Treatment methods 2.1 The control group was given conventional basic treatment of diabetes mellitus, and all patients were given diabetes education, strict and reasonable diet, appropriate exercise and reasonable selection of hypoglycemic drugs or treatment with insulin before treatment. 2.2 The treatment group was given Baihu plus ginseng soup plus reduction on the basis of the control group. Drug composition: 10g of Zhi Mu, 30g of gypsum (first decoction), 10g of licorice, 10g of round-grained rice, 10g of ginseng and 300ml of juice from water decoction, 150ml each time and taken orally twice a day, one dose a day. Add or subtract with evidence: increase the amount of gypsum for heavier thirst, add 15g of wu mei and 15g of geranium, add 15g of cunningham and 15g of raw earth for heavier depletion of yin. 10g of rhubarb for constipation, or 10g of mannitol. 15 days for a course of treatment above, and review blood sugar once a week. 3, efficacy observation 3.1 observation index ① Chinese medicine symptoms, signs improvement, according to the Chinese medicine evidence efficacy standards to determine. ②Blood specimen: fasting blood of elbow vein was taken before treatment and the next morning after treatment for the determination of fasting blood glucose. ③Fast blood glucose: glucose oxytocin method was used. 3.2 The efficacy standard refers to the efficacy standard of the “Chinese Medicine Internal Disease Treatment Routine”. Effective: clinical symptoms basically disappeared, FPG≤6.1mmol/L. Improved: main symptoms and relevant laboratory tests improved (FPG≤7.0mmol/L). Ineffective: No improvement in symptoms and laboratory tests after treatment. 3.3 Treatment results Chinese medicine clinical evidence efficacy: 28 cases (70%), 10 cases (25%), 2 cases (5%) invalid total effective rate of 95%. 4, typical case Li, male, 35 years old, oil field worker. He was diagnosed with type 2 diabetes mellitus in Jilin Oilfield General Hospital 2 months ago due to thirst, excessive drinking and eating, and was given controlled diet and glimepiride 2mg orally before breakfast, but his blood sugar was not well controlled. Diagnosis: disturbed heart, thirsty, thirsty, hungry, dry stool, yellow tongue coating, large pulse. His fasting blood glucose was 9.8 mmol/L, 15.2 mmol/L after breakfast, 15.8 mmol/L after lunch, and 14.6 mmol/L after lunch. Recipe: Bai Hu plus Ginseng Tang with reduction: Zhi Mu 10g, Gypsum (first decoction) 30g, Licorice 10g, Japonica rice 10g, Ginseng 10g, Pueraria Mirifica 15g, Radix et Rhizoma 15g, Rhubarb (later) 10g. 300ml of juice was decocted in water, each time 150ml was taken orally twice a day, one dose a day. Continue to take Glimepiride orally, the above plus reduction treatment for 15 days, the patient’s heart disturbed, thirsty and lead to drink, more food and good hunger, the symptoms disappeared, the second stool is normal, the tongue is light red, thin yellow moss, pulse string. Fasting blood sugar 5.5mmoL / L, 2 hours after meal blood sugar 7.8mmoL / L follow-up 2 months, the condition is stable. 5.Discussion The “thirst disease” in Chinese medicine belongs to the category of diabetes mellitus in Western medicine, and is a common and frequent disease in internal medicine. The traditional theory is that the pathogenesis is “Yin deficiency as the basis, dry heat as the standard”, and many doctors have adopted the treatment method of benefiting Qi and nourishing Yin, clearing heat and nourishing Yin.
