Male congenital malformation (hypospadias)

  Hypospadias: It is a common type of congenital malformation in males, mainly characterized by downward curvature of the penis and urethral defects. The opening of the urethra is not at the glans, but at the anterior, middle or posterior part of the ventral side of the penis, or in severe cases at the scrotum, and the urinary line is downward during urination. In severe cases, it is even impossible to distinguish the sexes because of the deformity in appearance!  Because it is a congenital malformation, it can usually be detected after birth. So, when is the best time to treat it? Surgery is usually best when the child is 2-5 years old for correction. During this time, the surgery is relatively safe, the child does not yet have full memory capabilities, the recovery is often good, and there is no psychological shadow on the child.  Many people would think that since it is a somatic surgery, the surgery would be simple, but it is not! Hypospadias is a disease that belongs to the quadrant of the discipline and requires the treating surgeon to have a wide range of abilities and surgical skills. It mainly includes perfect surgical experience and operating skills, surgical skills in urology, basic skills in pediatric surgery, and mainly plastic surgery. Internationally, many specialists have worked tirelessly on the surgical correction of this deformity and have summarized about 200 surgical procedures! However, for each child and each deformity, the real test is the surgeon’s ability to innovate.  Is hypospadias surgery successful or not? Four aspects need to be addressed: 1. whether the penis is fully straightened; 2. whether the urethra is reconstructed to an almost normal level; 3. whether there are any complications; and 4. whether there will be a residual defect in appearance.  Several major aspects of surgery are summarized including: Perfect preoperative surgical design.  Precise surgical operation.  Precise design of the reconstructed urethra.  Careful postoperative management.
