According to the report of the World Health Organization (WHO), the incidence of tumors in the world is now at an ever-increasing stage, and it is expected that by 2020, the number of new cases will reach 15 million per year. In China, malignant tumors have ranked first among the common causes of death. Due to the complexity of tumor diagnosis and the limitations of the current diagnostic level, especially in developing countries, a large number of patients are diagnosed in the middle and late stages, and have already lost the opportunity for curative treatment, so palliative care occupies an important position in most of the tumor patients. In addition, with the transformation of medical model from biomedical model to biopsychosocial medical model, the idea of tumor treatment is no longer purely pursuing the elimination or shrinkage of tumor entity and killing of tumor cells, but also taking into account the improvement of patient’s physical and mental symptoms, and the improvement of the quality of life, and the quality of survival, which is the new health indicator, has been receiving more and more attention from the people. The Fuzheng therapy of traditional Chinese medicine has greater advantages in improving body quality before and after surgery, radiotherapy and chemotherapy, promoting diet, enhancing hematopoietic function of bone marrow, elevating white blood cells, and prompting the injured organism to recover within a short period of time, as well as improving the disease-resistant ability of the organism, correcting and repairing pathological damages, prolonging the period of survival, alleviating the painful symptoms, and improving the quality of survival. Therefore, the Chinese medicine supportive therapy in the palliative care of malignant tumors is showing more and more important role. 1, the understanding of the disease mechanism Chinese medicine believes that the human body contains the resistance to disease – positive qi, that is, the immune function. The Neijing cloud: “positive qi within, the evil can not dry”, “the evil of the gathering, its gas must be weak. It can be seen that positive qi is to maintain the body’s normal life activities, resistance to external evil and disease prevention and treatment of basic substances. Whether it is strong or not is related to the human body’s life activities and the return of the disease. In this sense, the positive qi of Chinese medicine includes the immune function of modern medicine. Therefore, to enhance the immune function of the human body, improve the body’s ability to recognize foreign bodies and kill tumor cells, it is necessary to support the positive qi. And Chinese medicine has immunomodulatory effect, which can activate immune cells, improve the body’s immune function and play anti-tumor role. Radiotherapy is the use of ionizing radiation to induce the production of cells, tissues and organs to change the reaction, causing changes in the biological structure, with the destruction and prevention of cell division, to achieve the purpose of eliminating malignant tumors. However, at the same time, normal tissues and organs are damaged by radiation, especially the immune tissues or organs, causing redistribution of T-lymphocyte subpopulations and leading to immune dysfunction. According to Chinese medicine, radiation as a “fire poison and heat” injures yin and consumes qi, damages yin and burns the fluid, impairs the function of the spleen and stomach, and affects the source of qi and blood biochemistry, resulting in qi and blood damage, disorders of the spleen and stomach, deficiencies of the liver and kidneys, and the emergence of various diseases. Chemotherapy, on the other hand, adopts the cytotoxic effect of chemical drugs to kill cancer cells, but it also kills normal cells and damages many organs and tissues, thus lowering the immune function of the body. According to Chinese medicine, chemotherapeutic drug is a kind of hot and poisonous drug, which can injure yin and deplete qi, make the body’s positive qi damaged, and the spleen and stomach dysfunction, resulting in qi and yin injuries, qi and blood deficiency, spleen and kidney deficiencies, and residual cancerous toxins and stasis of blood with the chemotherapeutic drugs and poisons, which aggravate the depletion of the positive qi and also lead to the emergence of various syndromes. As mentioned above, the development of malignant tumors is based on the weakness of positive qi, which is manifested in the low immune function. Changes in the immune system and immune function are characteristic indicators of the pathological nature of spleen deficiency in TCM, and there have been many reports of abnormal cellular and humoral immune functions in patients with spleen deficiency. Many clinical observations and animal experimental studies have found that it can enhance the activity of peripheral blood NK cells and LAK cells, and increase the value of T3 and T4 lymphocytes, while conventional radiotherapy does not have this effect. Some studies have also found that spleen and kidney strengthening Chinese medicine combined with chemotherapy can improve the CD3, CD4, CD4/CD8 indexes and NK cell function of advanced tumor patients, thus improving the therapeutic effect and quality of life. All these show from another angle that the positive qi of Chinese medicine has a very close relationship with the immune function of human body. 2, therapeutic research The method of supporting positive qi in Chinese medicine is the method of supplementation, including benefiting qi, warming yang, nourishing blood, nourishing yin, etc. It is a therapy for the deficiencies of qi, blood, yin and yang in the organism. In specific clinical practice, it refers to replenishing the spleen and kidney as the focus, including the support of qi, blood, yin and yang. When correcting, the spleen and stomach are taken care of first, because if the source of biochemistry in the latter day cannot be properly transported, no tonic can play its proper role. If the spleen and stomach are not decayed, there will be a source of qi and blood biochemistry, and there will be a material basis for resisting disease and dispelling evil. In the late stage, the physiological functions of specific systems of the patient are insufficient, and their deficiencies and damages are also characterized, so the support of the corrections is also slightly focused. Just as the “Classic of Difficulties” puts forward: “the five viscera are supplemented”, the ancient cloud: “those who lose their lungs, benefit their qi; those who lose their hearts, and their camps and guards; those who lose their spleens, regulate their diets, and adapt to their cold and warmth; those who lose their livers, slow down therein; and those who lose their kidneys, benefit their essences”. Therefore, a series of symptoms such as loss of appetite, nausea, loose stools and fatigue of tumor patients are the manifestation of spleen deficiency, and the functions of spleen include multi-systemic functions such as digestion and absorption, water and salt metabolism, energy conversion, blood, nerves, endocrine, immunity and movement of modern medicine, etc. The deficiency of spleen runs through the whole process of tumor development and occurrence; and it is necessary to take care of kidney yang in the course of supporting the correctness of the spleen. Kidney is the foundation of the innate nature and generates essence, which is the root of human life. Tumor is a pathological product produced by yinheng, while yang deficiency is the pathological basis leading to yinheng. According to the theory of “Neijing”, Wang Bing proposed “benefiting the source of fire to eliminate Yin”, which means warming and tonifying the kidney yang, replenishing the innate yang qi and yuan yang to restrain the Yin qi, so that the tangible Yin evil can be naturally dispersed. Therefore, warming and tonifying kidney yang is the fundamental treatment method for tumor. In the palliative treatment of advanced tumors, supporting the correctness must be connected with the immunological research of modern medicine. As far as “positive” is concerned, the first thing that is closely related to it is the immune status of the tumor-bearing organism, which is well studied, among which the cellular immune function is particularly important, such as the vitality level of NK cells, cytotoxic T cells (CTL), monocyte macrophages and tumor-infiltrating lymphocytes that play an anti-tumor effect locally in the tumor; the second thing is also included in the anti-tumor process, which has a direct or auxiliary role in the anti-tumor process. The study of these related indexes is bound to provide the necessary modern medical evidence for judging the degree of deficiency of the body’s Zhengqi and guiding the treatment of Fuzheng in the near future, which will push the treatment of Fuzheng in TCM to a new height. TCM Fu Zheng method of treating tumor patients’ immune surveillance function to enhance the vitality of the human immune system, but also to protect and improve the body’s multi-faceted functions, improve the quality of patient’s survival and the reduction of toxicity and synergistic effect on chemo-radiotherapy. Fuzheng treatment has both traditional connotation including supporting positive qi, tonifying spleen and kidney, and adjusting yin and yang, qi and blood, and also covers the extension combining with the current characteristics of malignant tumors treatment. Whether it is combined with palliative radiotherapy or end-of-life care, Chinese medicine Fuzheng treatment occupies an important position.
