How can I differentiate between venous and arterial pulsatile tinnitus?

When performing peri-auricular and cervical palpation on the ipsilateral internal jugular vein, ask the patient to note their tinnitus; if the tinnitus is reduced or disappears, this suggests tinnitus of venous origin; tinnitus of arterial origin is not altered by the palpation. Alternatively, turn the patient’s head to the affected side and if the tinnitus becomes weaker or disappears, this is also suggestive of venous origin.
