Dry eye is a kind of eye discomfort, visual impairment, poor tear film stability, eye surface inflammation and its potential damage caused by multiple factors such as abnormal tear secretion. The main symptoms include dry eyes, foreign body sensation, burning sensation, itching sensation, blurred vision, etc. Persistent lesions can cause a decrease in corneal transparency and vision loss, thus affecting work, study and life, and even leading to blindness. With the popularity of computers and changes in lifestyle and habits, the incidence of dry eye disease is gradually increasing and showing a trend of low age development. How to effectively treat dry eye has become a research hotspot. At present, clinical treatment of dry eye often uses tear replacement and methods to maintain tear retention time in the eye as much as possible, which are passive methods to increase tears. Cyclosporine A, tear punctal embolization, and autologous submandibular gland transplantation have been effective, but they are also inadequate. The ideal treatment for dry eye is non-invasive and promotes active tear secretion from the gland, which is in line with the characteristics of acupuncture therapy in Chinese medicine. In recent years, we have applied acupuncture therapy for dry eye in our clinic and achieved good clinical efficacy, which is summarized below. 1.Treatment method Since the opening of the dry eye specialist clinic in the outpatient department of Shanghai Institute of Acupuncture and Meridian in 2004, after continuous clinical exploration, we have formed a more mature treatment plan of acupuncture with cupping therapy and auricular acupressure for the treatment of dry eye. In other words, acupuncture treatment is performed first, and the needle is left for 20 min, then cupping therapy is used, and the cupping is left for 5-10 min, and finally auricular acupressure is applied. Generally, the treatment is done once every other day, 3 times a week, and 10 times a course of treatment. Generally 1-2 courses of treatment can be effective. The main acupuncture points are Qingming, Zanzhu, Xinming II, Sun, Sibai, Hegu, Taichong, Guangming, Sanyinjiao, Baihui and Fengchi. The supporting points: liver and kidney yin deficiency: liver Yu, kidney Yu, Tai Xi; blood stasis: blood sea, Qu Chi; damp-heat congestion: Nei Ting, Feng Long; qi and blood deficiency: foot San Li, Guan Yuan; liver depression: qi stagnation: Nei Guan, Xing Ma, Shen Men. Cupping cupping is performed on DaZhi, Lung Yu, Di Yu, Liver Yu and Kidney Yu, and the cupping is left for 5-10 min to unblock the Qi and blood of the internal organs. Ear points were taken from Shenmen, Liver, Kidney, Eye, Eye 1, Eye 2, and subcortex, and magnetic beads were applied to one ear, alternating with both ears. Patients are instructed to press and stimulate themselves 4-5 times a day until the auricle is red and warm. For patients with severe dry eye, treatment can be combined with acupuncture point injection, taking Xinming II and Sun alternately, choosing Danshen injection or Angelica injection, both taking acupuncture points bilaterally, with the injection volume of 1mL per point. 2. Key points of regimen Keeping good working and living habits and paying attention to health care are effective means to prevent and treat dry eye. (1) usually pay attention to not overuse of the eyes, such as excessive computer, watching TV, etc., to develop more blinking work habits. Computer workers, pay special attention to avoid operating the computer for a long time, requiring a break of 3-5 minutes after operating the computer for 20-30min during treatment. When using the computer, adjust the distance and position of the monitor to the eyes, try to keep the distance above 60cm, position slightly below the eye level 10 to 20cm, so that the line of sight becomes overhead, and at the same time the brightness of the monitor should not be too bright. (2) Pay attention to the protection of the spleen and stomach, eat and live regularly, pay attention to diet, eat more fresh vegetables and fruits, increase the intake of vitamin A, B1, C, E, and should not eat too much spicy or stimulating food. (3) Pay attention to the regulation of emotions, maintain optimism, do not be under long-term stress; not too tired, pay attention to sleep enough not to stay up late, work and rest. (4) pay attention to environmental health care, less exposure to air conditioning and sand, smoke and dust environment, do not use hair dryers, do not smoke; place water near the seat to increase the surrounding humidity. Away from the city at the right time, to the countryside, green mountains and water, climbing high and far, breathing fresh air, is also very beneficial to the prevention and treatment of dry eye disease. (5) In addition, we should pay attention to appropriate exercise to enhance physical fitness. Moderate physical exercise can promote blood circulation throughout the body, increase the body’s metabolism, strengthen the blood supply to the eyes, improve the metabolism of the lacrimal gland, which will help the recovery of the disease. In ancient Chinese medical texts, dry eyes were called “white astringent disease” and “the water of the gods will dry up”, which belonged to the category of “dryness evidence”. The human body is an organic whole, and the internal organs are the center of the body’s vital activities. The eye, as an organ of the human body, has a close relationship with the five internal organs. Dry eye syndrome is due to the direct cause of localized deficiency of fluid in the eye. The root cause is mainly due to the dysfunction of the internal organs caused by the six sexes, seven internal injuries, inappropriate diet, fatigue, trauma, aging and other factors, resulting in the deficiency of fluid production or dispersion, resulting in the loss of moistening of the eye. The treatment is to open the orifices and activate the channels, regulate the internal organs, benefit the qi and nourish the yin, nourish the blood and moisten the eyes. The prescriptions pay attention to the distant and near points, and the techniques are mainly for toning and dredging, not forcing the supplementation and diarrhea. In the acupuncture prescription, the acupuncture points of Qingming, Zanzhu, Xinming II, Sun, and Sibai are taken locally to unblock the qi and blood of the eye meridians, open the channels and activate the channels, and stimulate the secretion of tears. The two points of Hegu and Taichong are compatible with each other as yin and yang meridians, up and down with each other, and the qi, blood, yin and yang organs are in tune with each other, with strong synergistic effects, which together play a role in tonifying qi and blood, clearing heat and dampness, and tonifying liver and kidney. Complemented by Fengchi, it strengthens the function of clearing the liver and brightening the eyes, clearing the channels and relieving pain. Guangming is a biliary meridian point, good for treating eye diseases. Sanyinjiao is the meeting point of the Sanyin meridian, which tonifies the spleen and stomach, benefits the liver and kidneys, regulates qi and blood, calms the mind and fixes the spirit. The application of Baihui point is based on the clinical experience of Professor Zhao Zuiying, a famous Chinese medicine practitioner in Shanghai, which emphasizes the role of toning the mind in acupuncture treatment; at the same time, considering that patients with dry eyes mostly have anxiety, we add Baihui point to the prescription to strengthen the calming effect of the mind. The fire cupping unblocks the qi and blood of the internal organs, and the ear point stimulation strengthens the synergistic effect, which together achieve the effect of opening the meridians, nourishing the blood and moistening the eyes, and stimulating the secretion of tears. Because dry eye is affected by many factors, the cure rate is low and easy to recur, so patients should pay special attention to the regulation of work life according to the aforementioned regulation points. In the clinical treatment of dry eye disease, we should pay attention to the comprehensive treatment, starting from regulating the qi and blood of the internal organs of the body, focusing on the addition and subtraction of evidence based on the basic formula of acupuncture, and the integrated application of acupuncture, ear acupuncture and cupping, which can help improve the clinical efficacy.
