With the improvement of people’s living standard, China is also entering the ranks of aging. With the increase in the number of elderly people, the incidence of geriatric diseases also increases. The incidence of senile cataracts is 60-70% among 50-60 year olds, while about 80% of those over 70 years old have cloudy crystals. So-called cataracts are cloudy crystals in the human eye and lead to vision loss. The etiology of cataract is not well understood, and it is a multifactorial disease, mainly due to physiological aging, malnutrition, radiation damage, systemic metabolic and endocrine disorders. It is not yet possible to effectively prevent cataracts, but reducing the risk factors for cataracts can reduce the occurrence of cataracts. For example, prevention of infrared, ultraviolet and sunlight damage to the eye, prevention and control of diabetes, renal insufficiency and severe diarrhea, etc. Topical and systemic medications for the eye should take into account the factors that induce crystal clouding. The severity of cataracts varies and so does the degree of impact on vision. Geriatric cataracts can be broadly divided into early and mature stages. Early stage cataracts are usually treated with medications because the degree of crystal clouding is mild and the effect on vision is also mild, but these medications only slow down the rate of crystal clouding to a certain extent and do not fundamentally relieve the clouding. The final result of cataract development is to cause visual impairment, that is, the mature stage, when surgery is the only effective means. Many elderly people who have cataracts have a fear of losing their eyesight, so they seek medical treatment everywhere, such as acupuncture, massage and compresses, etc. These methods currently have no definite efficacy and may have side effects. The author has seen the dislocation of the crystal caused by massage treatment for cataract, and the vision is reduced compared with that before treatment, which brings difficulties to cataract surgery as well. Therefore, experts call on cataract patients to go to regular hospitals and not to be deceived by some charlatan doctors. Cataract surgery is to remove the cloudy crystals in different ways. As the cloudy crystals are removed, the eyes are missing a part similar to a magnifying glass, and in recent years, artificial crystals have been implanted to replace the original crystals and to restore the eyesight in a real sense. Therefore, cataract extraction combined with IOL implantation is usually used to treat cataract in clinical practice nowadays. The methods of cataract extraction include modern extracapsular extraction, small incision cataract extraction and ultrasonic emulsion cataract extraction. In 1966, American experts developed the world’s first ultrasonic cataract extraction device, which was used in animal experiments and achieved success. After more than 30 years of continuous improvement and enhancement, this latest surgical method representing the modern concept has become the leading procedure for cataract surgery in developed countries. Ultrasound emulsification technology is one of the fastest developing and most high-tech clinical techniques in recent years, which integrates optical, mechanical and electrical high technologies. Ultrasound emulsification uses ultrasonic shock to directly crush cloudy crystals into celiac disease and aspirate them out, so that only 4-6L incisions are needed for the surgery, which not only reduces the side effects of the surgery, but also improves the results. Cataract ultrasound emulsification surgery uses surface anesthesia technology, which can be anesthetized without the use of injections. The surgery takes only 5-10 minutes and the eye band can be opened on the second day after surgery. In the past, traditional cataract surgery required the cataract to reach maturity before the surgery could be performed, so it was a long process for cataract patients to wait. During this period, if the crystal swells, glaucoma will occur, which will make the surgery more difficult in mild cases and cause irreversible loss of visual function in the affected eye in severe cases. Ultrasound emulsification surgery advances the operation time and can be considered when the visual acuity is below 0.5, which greatly improves the quality of life of patients and truly brings a blessing to cataract patients. Regarding the recovery of vision after cataract surgery, if the vitreous and retina of the affected eye are normal, the vision after surgery can be restored to normal, on the contrary, if the vitreous and retina of the affected eye are diseased, the vision after surgery will not reach normal. Therefore, ultrasound examination of the eye and electrophysiological examination of the retina before cataract surgery are very necessary because these results can objectively reflect the condition of the vitreous and retina and predict the effect of surgery when the fundus cannot be seen with cloudy crystal.
