Recently, the Department of General Surgery of Anyang Regional Hospital cooperated with the Interventional Center and successfully applied interventional therapy to save a patient with mesenteric artery trunk embolism. Mesenteric artery embolism (MAE) is an extremely critical acute abdominal disease in clinical practice. Although its clinical incidence is not high, it has a rapid onset, progresses rapidly, and is dangerous, easily leading to extensive ischemic intestinal necrosis. The rate of misdiagnosis and death is high. The mortality rate has been reported in the literature to be as high as 70% – 90%. A few days ago, this patient was sent to the general surgery department of a regional hospital with epigastric pain and was urgently examined 3 hours after admission for superior mesenteric artery vascular ultrasound, which showed superior mesenteric artery ischemia. The patient had a sudden onset of severe abdominal pain at around 11:00 pm. Because the patient has a history of atrial fibrillation, had a heart bypass and pacemaker surgery, the doctor on duty, Dr. Yan Ling, immediately consulted the hospital’s assistant director, Dr. Jia Xiaoqiang, director of major surgery after the initial examination. Dr. Jia was the first to arrive at the ward late at night in the cold, and Dr. Yan Ling checked the condition. According to the patient’s condition and medical history, it is suspected that a small thrombus or plaque dislodged from the atrial appendage leads to mesenteric artery vascular embolism, so he contacted the interventional department director Gao Yi and Dr. Zhang Qiang for an emergency consultation, suggesting emergency mesenteric artery angiography and emergency interventional treatment if necessary. At 0:30 a.m. the next day, six hours after the patient was admitted to the hospital, the interventionalists performed the interventional treatment under the guidance of Dr. Jia Xiaoqiang. The imaging showed that the main trunk of the superior mesenteric artery was occluded at the beginning of the mesenteric artery, and the distal vessels were not visualized. The patient’s diagnosis was clear: acute thrombosis of the superior mesenteric artery. After discussion with the family, the interventional thrombolysis procedure was performed. Under DSA surveillance, the thrombolytic catheter was left in the distal superior mesenteric artery after great efforts, and 300,000 units of urokinase was slowly pushed in. After the patient returned to the ward, he continued to pump urokinase slowly. 39 hours after the catheter was left in place, the superior mesenteric artery was completely open and the interventional thrombolytic treatment was successful. The patient’s pain symptoms completely disappeared and he was safely discharged from the hospital several days later. Superior mesenteric artery thrombosis has a high mortality rate, and the diagnostic level and speed of treatment of the receiving physician are necessary to ensure the patient’s life. The quality of life of the patient is greatly diminished after intestinal resection, which is necessary to save the patient’s life. Contact thrombolysis of acute thrombosis is an interventional therapy in which a thrombolytic catheter (surrounded by dozens of side holes) is placed directly in the blood vessel where the thrombosis occurs, and thrombolytic drugs such as urokinase are directly applied to the thrombus through the catheter, which accelerates the dissolution of the thrombus. This therapy is not only applicable to superior mesenteric artery thrombosis, but also to intra-arterial thrombosis of the limbs, intracerebral artery thrombosis and intravenous thrombosis. This case is the first one successfully treated by interventional therapy in Anyang Regional Hospital. It is less traumatic for the patient, quick recovery and good prognosis, and has been well received by the patient and his family. The correct diagnostic treatment by Dr. Jia Xiaoqiang and Dr. Yan Ling and the timely and skillful surgical treatment by the interventionalists led by Director Gao Yi not only won the patient’s life, but also the quality of life later. The successful treatment of this case has also raised the diagnosis and treatment of mesenteric artery embolism, a difficult disease, to a new level in Anyang Regional Hospital.
