Prostate cancer is a common malignant tumor in elderly men, and its incidence in the United States has become the first tumor that endangers men’s health. In China, with the increase of per capita life expectancy, changes in lifestyle and diet structure, and the widespread clinical use of prostate-specific antigen screening, the clinical incidence of prostate cancer has been on the rise. Due to the strong tissue infiltration and easy early metastasis of this disease, many patients are already in advanced stages when diagnosed clinically. Although most patients respond well to endocrine therapy at the beginning of treatment, after a median time of 14-30 months, almost all patients will develop hormone-non-dependent prostate cancer, making the disease progressive and then fatal, and most of the currently available western medical therapies are unable to achieve further efficacy. Therefore, the treatment of patients with advanced prostate cancer has become a challenge for clinicians. At present, although modern medicine has done more research in this area, the results are still not significant and the side effects are relatively large. Chinese medicine practitioners have applied Chinese medicine in the treatment of various advanced tumors, and have gained a lot of experience and consensus in long-term clinical practice and achieved certain efficacy. Some of these experiences are worthy of our reference for the treatment of patients with advanced prostate cancer. The disease of prostate cancer is the result of the conflict between the damage of the evil and the resistance of the positive energy. In late stage prostate cancer patients, evil and positive have been fighting for a long time, and the deficiency of positive qi has led to the loss of positive qi, imbalance of yin and yang, dysfunction of internal organs, stagnant guest evil, phlegm coagulation and poison gathering, mutual agglomeration and tumor formation, and the growth of cancer tumor will further deplete the positive qi, and the development of cancer tumor will be facilitated by the failure of positive to curb evil. Therefore, deficiency of vital energy is the intrinsic cause of advanced prostate cancer. It can be seen that the process of prostate cancer is a process in which the body’s positive qi is weak and the evil qi takes advantage of the deficiency to invade gradually. Prostate cancer mostly occurs in elderly men, and the deficiency of kidney essence and kidney qi is the physiological characteristic of elderly men, because the clinical diagnosis of prostate cancer is mostly in advanced stage, the body suffers from cancer attack for a long time, which consumes essence and blood and damages vital energy; when the tumor invades the bladder or bone metastasis, it will cause blood in urine and anemia, which makes people’s physique weaker and often have deficiency of both qi and yin. Especially after surgery and radiotherapy, prostate cancer patients have been killed and their qi and yin are greatly injured. The clinical manifestations of advanced prostate cancer patients are mostly due to the involvement of the spleen and kidneys. In elderly men, the deficiency of the kidney is a major cause of the lack of yin and yang in the five viscera; the cancer often has bone metastasis, which directly destroys the function of the kidney as the master of bone and water. The spleen is the main organ of transportation, clearing, and blood control. Patients with prostate cancer often suffer from weakness, low blood intake, decreased white blood cells, red blood cells, platelets, and hemoglobin, constipation (hard at first, then soft, or even unformed), and edema of the lower extremities. Therefore, the treatment of advanced prostate cancer should be one of the main TCM methods. At the same time, considering the fact that patients with advanced prostate cancer are mainly suffering from positive deficiency and the cancer has spread, complete elimination of the tumor is an impossible task, if we make a big effort at this stage and pursue too many means to kill the tumor, it will easily cause more damage to the patient’s body and even fatal. Therefore, it is more realistic to adjust the treatment idea, adopt the method of supporting and cultivating the root, try to improve the patient’s own resistance, improve the appetite, reduce the clinical discomfort of the patient, not to overemphasize the removal of cancer cells, and pursue the relatively harmonious coexistence with the cancer. We have adopted this idea in our clinical practice, and while maintaining the conventional treatment in western medicine, we have improved the quality of patients’ survival by regulating the spleen and stomach, taking into account the symptoms of weakness of the righteousness of patients in advanced stages. The theoretical basis is that people are based on water and grain, and water and grain essence is the material basis of human life activities. According to Chinese medicine, the spleen and the stomach are the basis of the latter, and the stomach is responsible for receiving, the spleen is responsible for transporting and transforming, and the spleen rises and the stomach descends, together completing the incorporation of food and the transportation of water and grain essence. In Su Wen? It is written in the “Treatise on the Meridians and Vessels” that “when drinking enters the stomach, the essence overflows and is transferred upward to the spleen; the spleen disperses the essence and returns upward to the lungs; the water channels are regulated and transferred downward to the bladder.” Thus, water essence can be dispersed in the limbs and the five meridians run in parallel throughout the body. Qi and blood play an important role in human life activities, and the spleen and stomach are the source of biochemical Qi and blood. The body’s qi is generated by the spleen and stomach. The vital energy is the driving force of the body’s vital activities, which is generated by the innate essence and is constantly nourished and replenished by the acquired essence. The “Spleen and Stomach deficiency is not open to the nine orifices” says that “true qi, also known as yuan qi, is the essence of the first body, and cannot be nourished without stomach qi”. The spleen and stomach are located in the middle jiao of the human body, and they are the hub of the whole body’s qi up and down. The spleen and stomach should be healthy for transportation, and the stomach should be harmonious. The spleen and stomach is the essence of the day, in the center of the jiao to irrigate the four sides, the five viscera and the six internal organs are all by the spleen and stomach gas to breathe. Spleen Qi develops, then the liver Qi rises, the kidney water can rise. The stomach qi descends, the lung qi can be purified and lowered, the heart fire can therefore be lowered. Therefore, those who are good at treating the spleen and stomach can regulate the five organs of a person by regulating the spleen and stomach. Zhou Shenzai once said, “If all diseases are not cured, we must look for the spleen and stomach, so that there is no loss. Spleen and stomach a deficiency, the four organs are lifeless, so the disease day long carry on. Everything is born from the earth, but also from the earth and return, the kidney is better than the spleen, this is also said. There are many people who have not recovered from their illnesses, but have found the spleen and stomach and recovered.” The Spleen is the source of biochemistry and the root of the five organs. The Spleen is the basic formula for supporting the righteousness and cultivating the root, and by benefiting the Qi, it can produce blood and essence, and it can also consolidate the detachment. The formula of Si Jun Zi Tang, as the basic formula, can promote blood and sperm, and also consolidate detachment. As the saying goes, “If the righteousness exists inside, evil cannot dry up”. The method of benefitting Qi in Si Jun Zi Tang can be adapted and used in a variety of ways. For example, to benefit qi and disperse stagnation, use Isogong San (Si Jun Zi Tang plus Chen Pi); to benefit qi and dissolve phlegm, use Liu Jun Zi Tang (Si Jun Zi Tang plus Chen Pi and Han Xia); to benefit qi and dispel dampness, use Ginseng Ling Bai Zhu San (Si Jun Zi Tang plus Lentil, Coix Seed, Yam, Lotus Seed, Sand, and Radix Platycodon); to balance qi and yin, use Sheng Wei San or Zeng Yu Tang combined with Si Jun Zi Tang; for deficiency of both qi and blood, use Guei Shao Liu Jun Zi Tang, or Bajhen Tang (Si Jun Zi Tang plus Si Wu Tang); for deficiency of both heart and spleen, use Guei Shao Six Jun Zi Tang, or Bajhen Tang (Si Jun Zi Tang plus Si Wu Tang). The main reason for this is the deficiency of the heart and spleen. Among the patients with advanced prostate cancer that I have seen, the above-mentioned ideas have been used to guide the selection of prescriptions and medicines, which have effectively reduced the symptoms of constipation, edema, loss of appetite and weakness in many patients and partially improved their quality of life. At the same time, combined with the patient’s physical tolerance, some detoxification and anti-cancer drugs can be added at appropriate discretion to directly suppress the tumor. However, it should be emphasized that exogenous androgen supplementation can significantly promote the condition of prostate cancer patients, and some Chinese medicines, especially those for kidney yang, contain androgen-like substances. In one case, I treated a patient with advanced prostate cancer because he thought that traditional Chinese medicine had no side effects, and he took a prescription for kidney-strengthening Chinese medicine to treat bone metastasis caused by prostate cancer. The testosterone level slowly decreased to the depot level only 3 weeks after the depot surgery, and the patient eventually died because of the drastic changes in his condition that led to multiple organ failure. A large number of previous studies have also confirmed that patients with malignant tumors have low immune function, and the application of traditional Chinese medicine to support and cultivate the essence of the treatment can indeed partially improve the patient’s thalamus-pituitary-adrenal cortex function, which can partially reduce the side effects of radiotherapy and chemotherapy and protect the bone marrow hematopoietic function. For patients with advanced prostate cancer, although Fu Zheng Pei Ben Chinese medicine cannot cure the tumor, it can partially improve the patient’s clinical symptoms, improve the quality of life, and truly survive with the cancer.
