If the patient themselves have presented with a previous history of thoracic spine pain that has been treated and then recurred, there may be an organic lesion of the thoracic spine, such as a herniated disc or slipped thoracic spine, etc. Just because a previous MRI was fine does not mean that there is no organic lesion, it is just that at the time of the examination the lesion was not yet severe and can appear on imaging as an inconspicuous or non-existent lesion that needs to be re-examined in time to exclude the problem.
