Chronic lymphocytic leukemia

Overview Chronic lymphocytic leukemia is due to tumor-like proliferation of lymphocytes and is characterized by an accumulation of mature forms of lymphocytes in the body resulting in an increase in lymphocytes in the blood and bone marrow, enlargement of lymph nodes, liver and spleen, and eventually involvement of tissues other than the lymphatic system. Etiology The etiology is unknown. Symptoms I. General symptoms: insidious onset, slow progression, fatigue, malaise and emaciation caused by the tumor itself. Itchy skin, hyperpigmentation, erythema, exfoliative dermatitis and herpes zoster. The most common infections include respiratory tract, skin, gastrointestinal tract, urinary tract, sepsis, etc. Liver and spleen lymph node enlargement: Superficial lymph node enlargement is the most common sign of slow gonorrhea, which can change from small to large, from less to more, from local to generalized as the disease progresses. IV. Skin damage. Examination i. Blood picture: Total white blood cell count is elevated. Bone marrow picture: Bone marrow is actively proliferating, lymphocytes are significantly increased, accounting for more than 40%, and the morphology is basically the same as that of peripheral blood, and primitive lymphocytes usually do not exceed 1-2%. Immunological examination. Treatment I. Symptomatic treatment. Infection should be avoided, and if there is infection, it should be treated actively. High-dose intravenous administration of C-immunoglobulin can significantly reduce bacterial infections in patients. Corticosteroids are available for immune hemolysis. In cases of hypersplenism, splenectomy is beneficial to improve thrombocytopenia. Second, chemotherapy. The most commonly used drug is lumecoxibine, which is the standard therapy for slow gonorrhea. CHOP can be used for advanced disease or if the above regimen is ineffective. Radiotherapy. It can be used for local irradiation of giant lymph nodes and giant spleen, but the efficacy of small doses of systemic irradiation is limited.
