How to deal with incomplete semen liquefaction in male infertility

  Clinically, many male patients who visit the clinic for infertility have semen analysis suggesting incomplete liquefaction, how should we respond in the face of incomplete semen liquefaction?  First of all, we should understand the process of temporary coagulation and gradual liquefaction of semen. After the semen is ejaculated, it is soon in the form of a gelatinous clot, i.e. the coagulation of semen. Later, within 15-30 minutes, it gradually changes to liquid state, which is the liquefaction of semen. The coagulation of semen is related to the seminal vesicle gland, which secretes a protein-like substance that causes semen to coagulate. If the semen is not coagulated due to abnormal function of the seminal vesicle gland, the uncoagulated semen will soon flow out of the vagina, resulting in a decrease in the number of sperm, which can easily cause infertility or low fertility. After semen coagulation, semen gradually begins to liquefy, usually within 15-30 minutes. If it does not liquefy or liquefies incompletely for more than an hour or several hours, it is considered abnormal. The non-liquefaction of semen is related to the prostate gland, which contains a high concentration of protein hydrolase and fibrinase, which are the promoters of semen liquefaction. Normal liquefaction is necessary for the smooth passage of sperm through the female reproductive tract and their eventual union with the egg. If semen liquefaction is incomplete, it restricts the sperm from swimming, which means it causes low sperm motility and can likewise cause infertility or low fertility.  The causes of semen non-liquefaction are now considered by research to be mainly related to prostatitis and other reproductive system infections. The reason for this is that the prostate secretion is decreasing. The first part of the ejaculated semen mainly comes from the prostate gland and contains the enzymes that liquefy semen. If this part is missed during semen collection, it can easily cause artificial semen non-liquefaction.  The treatment of semen non-liquefaction is mainly targeted treatment, such as anti-inflammatory treatment, and also a large amount of vitamin C and enteric aspirin can be taken, or hyaluronidase can be injected; drugs to reduce the viscosity of sputum can also be tried; in addition, liquefying agents can be used for in vitro treatment of semen, commonly used liquefying agents include Alevaire cleanser, pancreatic deoxyribonuclease and Locke suspension containing 5% alpha-amylase. These liquefying agents do not affect the motility and viability of sperm. They can be used as follows: 60ml of Alevaire for precoital vaginal irrigation; or 1ml of suspension containing 5% alpha-amylase injected into the vagina with the hips elevated for 30min after intercourse during ovulation; or 50mg of alpha-amylase made with cocoa butter and inserted into the vagina immediately after intercourse. Of course, artificial insemination on the basis of in vitro optimization of semen is also one of the optional measures.  Chinese medicine has its outstanding therapeutic value for unexplained infertility and idiopathic weak spermatozoa that manifest as incomplete semen liquefaction. Contemporary Chinese medicine calls semen non-liquefaction as “semen stagnation”. According to TCM theory, the liquefaction of semen depends on the qi-chemical effect of yang qi, as the saying goes, “Yang transforms qi, yin forms”. The etiology includes: 1. congenital or acquired loss of kidney yang, resulting in loss of qi-transformation; 2. internal invasion of cold and dampness that damages yang qi and prevents it from transforming qi and moving water; 3. damage to kidney yin caused by various factors and internal burning of the seminal chamber by deficiency fire, resulting in thick semen that is difficult to transform; 4. external dampness and toxicity or internal dampness and heat that is injected into the seminal chamber, fumigating it and blocking it with dampness and heat; 5. prolonged illness, trauma or withholding semen from ejaculation, resulting in internal obstruction of stagnation and blockage of qi flow and liquefaction. Inadequate liquefaction. The treatment of this disease is based on the principle of helping the righteous and dispelling the evil and restoring the function of qi and chemistry. The righteousness includes warming the kidney yang and nourishing the kidney yin, while the evil can be dispelled by relieving dampness and turbidity, warming the meridians and dispelling cold, clearing dampness and heat, regulating qi and activating blood. All in all, TCM treatment emphasizes evidence-based treatment (grasp the main contradiction) and holistic treatment (holistic view), through correcting the imbalance of the organism, regulating the body to improve the internal environment for sperm survival, and finally achieve the purpose of promoting normal semen liquefaction and improving sperm quality.
