The most common cause of nail plate delamination and unevenness is damage to the nail plate and proximal nail folds. Examples include frequent hand soaking in water or frequent contact with detergents, frequent picking of the nails and nail folds, eczema, psoriasis, flat moss etc. If the full nail plate is abnormal in texture, consider onychomycosis and various conditions affecting the function of the nail matrix, such as lichen planus, psoriasis, eczema, pemphigus, and limb circulation disorders.
