It was a day in early November when Dang was sitting on his hometown’s electric car when a speeding car knocked them out, leaving the hometown with minor injuries but Dang fainted. 120 emergency vehicles flew him to Suzhou High-tech District People’s Hospital, and through a cranial CT examination (see Figure 1), Dang had massive intracranial bleeding! In addition to an epidural hematoma in the right temporal region, there was also contusion of brain tissue in the left temporal lobe. At this time, the patient had already developed brain herniation and had to undergo craniotomy as soon as possible to remove the hematoma in order to save the patient’s life! Our hospital urgently opened a green channel and sent the patient to the intensive care unit (ICU) without paying any fees, and actively carried out all the preparations before the operation. At this time, the patient’s sister also arrived at the hospital after receiving a phone call from her hometown, and after completing the procedures such as signing, the party was sent to the operating room. The operation went smoothly, and the postoperative review of the cranial CT (see Figure 2) showed that the epidural hematoma on the patient’s right side was completely removed, the cranial flap was also fixed back, and the left temporal lobe brain contusion was not significantly enlarged, and the treatment was continued in the ICU under supervision. On the first day after the operation, the patient was a little conscious but still did not wake up, was there a new intracranial hemorrhage? A review of the cranial CT (see Figure 3) verified that the doctor’s guess was correct: the patient had a large contusion of the left frontal and temporal lobes with hemorrhage, and another operation was necessary to remove the contusion foci and hemorrhage. After obtaining the understanding and consent of the family, the second surgery was performed successfully. The broken brain tissue and blood clot were removed together, but the brain tissue swelling was so severe that it was significantly higher than the edge of the bone window, so the bone flap could not be put back, and the meninges had to be sutured with reduced tension to facilitate the postoperative decompression of the brain tissue swelling. The postoperative review of the CT film showed that the patient had a large area of cerebral edema in the brain tissue (see Figure 4). Through careful treatment in the brain surgery and ICU, the patient slowly became conscious after one week, but had severe aphasia, and could not understand others’ speech nor could he speak himself, but could only make simple “ah, ah” sounds. This is medically called “mixed aphasia”, which means that the patient has both sensory and motor aphasia after severe damage to the speech center. Two weeks later, as the cerebral edema subsided, the patient was gradually able to speak, but still could not understand our speech. When we communicated to him, we had to write down the questions on paper. The patient’s memory recovered well, and as long as you showed him the name on his induction card once, he would affectionately call out your name the next time he saw you, although it was not yet very clear. But looking at his son who had come back from the dead, the young party’s elderly father was still moved to tears. Through nearly a month of careful treatment, Xiaodang’s language function has now been basically restored, and there is no longer any obstacle to communication with his family and doctors. Recently, during every check-up, Xiaodang always forgets to ask, “When can I go home? Of course, the party can already be discharged home to recuperate, but the processing of the traffic accident has not yet been completed, the family requested a few days to postpone the discharge procedures, otherwise, the homecoming party would have been able to return to the longed-for hometown.
