After radical prostate cancer surgery, when the catheter is first removed, about 35% of patients will have a small amount of urine leakage from the urethra when the abdominal pressure increases (sneezing, laughing, coughing, exercising, etc.), and most patients will regain normal urinary control in the weeks to months after surgery. Therefore, if there is mild incontinence after extraction, do not be overly stressed because the incontinence will improve or recover with time. All patients have to do is to insist on lifting exercises to exercise the muscle groups of the pelvic floor and be confident that they will regain urinary control. The purpose of pelvic floor muscle function restoration exercises (Kegel, Kegel maneuver) is to strengthen the tone of the pelvic floor muscles and the exercise helps in the recovery of urinary incontinence. The maneuver is accomplished by contracting the control urethral sphincter, as if one were trying to hold urine or inhibit a bowel movement, and works mainly the muscles behind the scrotum and in front of the rectum. Patients should start practicing this maneuver before the procedure. 1. Identify the pelvic floor muscles Contract the anus (similar to the process of final interruption of urination or inhibition of anal discharge), imagine how you feel when you interrupt urination and avoid contracting the muscles of the abdomen, buttocks or legs. Feel your abdomen with your hands during the exercise, if you feel tension in your abdominal muscles, then the movement is not correct. 2. How to perform the exercise Contract the anus forcefully while standing, sitting or lying down, hold for 2-3 seconds, then relax for 2-3 seconds, do not hold your breath, breathe normally during the exercise, 20 sets of exercises each time, at least 30 times a day, increase the duration of the contraction by 4 seconds in the second week after the catheter is removed, and increase the duration of the contraction to 5-10 seconds in the third week after the catheter is removed and beyond. You will feel an improvement in your ability to control the flow of urine. Usually, it is better to do the exercises in the morning. As the pelvic floor muscles strengthen, gradually increase the duration of retraction to 10 seconds. The key to pelvic floor exercises is the correct method and persistence.
