How much do you know about preventing and treating blindness?

Every year, June 6 is National Eye Care Day. According to statistics, there are about 4-5 million cataract patients in China, and if the incidence of cataract in Europe and the United States is 1/1000 of the population, there will be 1.2 million new cataracts in China every year. With the accelerated pace of aging in China, this number is still increasing, while only 500,000-600,000 cataract surgeries can be completed in China every year. Therefore, there are still 500,000-600,000 cataract patients in China who go blind every year due to lack of treatment, and a new case of blindness occurs almost every minute. Therefore, although a lot of fruitful work has been done by the government at all levels and many social groups over the past 20 years to prevent and treat blindness, China still has a long and difficult road to go to achieve the goal of “Vision 2020, the right of everyone to see, and the elimination of avoidable blindness”. The most common blindness-causing eye diseases are cataracts, glaucoma and corneal diseases, which are the three main types of eye diseases that we need to prevent and treat. If properly treated, blindness can be completely avoided. Cataracts are divided into different types according to different causes: 1. senile cataracts, which account for more than 80% of cataracts and are degenerative changes in old age. 2. congenital cataracts, where crystal clouding already exists at birth, caused by genetic factors or maternal infection with viruses or drug poisoning in early pregnancy. 3. traumatic cataracts, caused by trauma to the eye. 4. concurrent cataracts, caused by eye diseases or systemic diseases, such as uveitis membrane p glaucoma p diabetes mellitus p nephropathy, hypocalcemia, etc. For cataract alone, there are so many things we need to do. First, governments at all levels should actively participate. First of all, we should raise awareness in our mind, raise it to the height of the physical quality of the whole Chinese nation and the eradication of poverty in our country to understand it, and increase the effective investment in preventing and treating blindness. Most of the cataracts in China are found in rural and remote areas, where the poor economic base and lack of medical and health resources prevent a large number of blind cataract patients from receiving treatment. This requires the local government to adopt various methods to raise funds to strengthen the training of ophthalmologists in local hospitals and to purchase necessary equipment for blindness prevention and treatment. The relevant government departments should strengthen the coordination. Due to the system and other reasons, most of our doctors who can perform cataract surgery are concentrated in the central hospitals in cities, and many high-grade equipment (such as operating microscopes and cataract ultrasound emulsifiers) are also concentrated in these large hospitals. Thus, on the one hand, cataract patients in the most rural and remote areas cannot afford cataract surgery or do not know which hospital or doctor can perform cataract surgery due to financial reasons. On the other hand, large hospitals in the cities do not have enough cataract patients to do it, resulting in hidden waste and idleness of equipment and talents. One set of data can illustrate this point: in Europe and America a doctor who does cataract ultrasound emulsification does 3000-5000 surgeries a year, while in China there are not many doctors who can do 1000 surgeries a year in provincial level hospitals. Most cataract surgery specialists in ophthalmology perform only a few hundred surgeries a year. Therefore, the government should formulate relevant policies and adopt medical cooperation, funding and support to strengthen the cooperation and exchange between large hospitals and community township hospitals, so that people can make the best use of their talents and resources. Second, the media should actively participate. Publicize the correct view of blindness prevention and treatment and resist pseudoscience. At present, there is no ideal drug treatment for cataract, and surgery is the only way to cure cataract at root. Due to the drive of economic interests, many newspapers and TVs are now extolling the idea that medicine can cure cataracts, or even put a small piece of something on the eye and claim that it can cure cataracts, which are deceptive nonsense. These people seize the patient’s fear of surgery p fear of open surgery and expand the complications of surgery infinitely to achieve the purpose of cheating people’s money. Therefore, the first thing to do is to do a good job in the prevention and treatment of blindness is to make a big article of popular science propaganda, so that patients have a basic correct understanding of cataract. They should be informed that the current cataract ultrasound emulsification technology is a very safe and effective surgery without any pain, the incidence of surgical complications is very low, and the rate of IOL implantation is very high. After this technology was carried out independently in our hospital since 1997, the IOL implantation rate reached 99.6%. Third, colleagues in the ophthalmology field should actively participate. Preventing and curing blindness is our important task for quite a long time in the future, and it is also the main task of our ophthalmologists. Ophthalmologists, especially those who are engaged in cataract surgery, should be aware of the great responsibility they have. On the one hand, we need to strengthen the publicity. 1, to promote some basic general knowledge of blindness prevention and treatment to patients, so that patients will take less detours. 2, to strengthen their own publicity, to let more patients understand you p familiar with you p trust you. On the other hand, you should practice your basic skills. For a patient, who may have only one surgery in his life, success and failure are very critical to him. A failed surgery may bring pain to the patient’s life, which is undoubtedly a very important thing for both the patient and the doctor. Ophthalmologists should understand that cataract surgery is not just a surgery to restore sight, but a surgery to reconstruct normal vision. Therefore, each cataract surgery should be treated as a work of art to be carefully carved and perfected. Only in this way will they be worthy of the title of “messenger of light”.
