Gout is a crystalline arthropathy caused by disorders of purine metabolism and/or reduced uric acid excretion, which can manifest as acute recurrent arthritis, gout stone formation, gouty chronic arthritis, uric acid nephropathy, uric acid urinary tract stones, and in severe cases, joint disability and renal insufficiency. Acute gouty arthritis is classified as “paralysis” and “heat paralysis” in Chinese medicine, and Chinese medicine has greater advantages in the treatment of gouty diseases. Our department has achieved good results in treating acute gout attacks by combining Chinese and Western medicine. We have avoided the toxic side effects caused by the application of modern drugs, while relieving patients’ pain efficiently and quickly and shortening the duration of the acute phase. The clinical experience in the treatment of acute gouty arthritis with Chinese medicine is presented as follows: Objective: to rapidly terminate joint pain, relieve joint redness, swelling and fever symptoms, reduce blood uric acid, restore normal joint function and improve the patient’s quality of life. 1. General treatment 1. Diet: (1) Adjust the diet to be high in carbohydrates, moderate in protein and low in fat. Control the total calorie intake to prevent overweight. (2) Strictly limit high purine foods, such as animal offal (heart, liver, kidney, brain, etc.), seafood (fish, shrimp, crab, etc.), soy products (tofu, soy milk, soybeans, peas, etc.), alcohol (white wine, red wine, beer, etc.), yeast, etc. (3) Drink more water, about 2000ml or more per day for adults, and eat more alkaline foods. Such as vegetables, fruits, etc. (4) Avoid overeating, alcoholism, cold and damp, excessive fatigue and other triggering factors and active treatment of related diseases. (2) Rest: Rest in bed, elevate the affected limb and avoid weight-bearing on the joint. Chinese medicine treatment 1, Chinese medicine external treatment: treatment: clear heat and detoxification, reduce swelling and pain. Scope of application: acute gouty arthritis, acute attacks of chronic gouty arthritis. Symptoms: joint redness, swelling, heat, pain, unfavorable joint flexion and extension, etc. Chinese medicine identifies those with rheumatic heat paralysis. Usage: Take Qingbi San (rhubarb, etc.) in appropriate amount of physiological saline to make a paste and apply it externally to the swollen and painful joints, with a slightly larger scope than the swollen and painful scope. Apply the auxiliary material externally to prevent drying, 1-3 times a day. 2.Chinese medicine soup: prescription: rheumatism No.1 formula plus or minus. Treatment: Clearing heat and circulation, dispelling wind and removing dampness. Prescription: Coix Seed 10-15g; Fangji 10-15g; Tu Fu Ling 10-15g; Huang Bai 10-15g; Luo Shi Vine 10-15g, etc. Addition and subtraction: for heavy dampness, add Che Qian Zi and Slippery Stone; for spleen deficiency, add Astragalus and Atractylodes; for heat, add Forsythia and Zingiber officinale. Dosage: Decoct the above herbs with water and take 200ml of juice twice a day. Western medicine treatment 1, sodium bicarbonate tablets, 0.5-1.0, three times a day orally, so that the urine PH value to maintain at about 6.5; at the same time to drink more water to ensure adequate urine volume. It is contraindicated for patients with gastrointestinal diseases or diseases of water and sodium retention. 2.Patients with poor effect of external application of Chinese medicine can add non-steroidal anti-inflammatory drugs, such as diclofenac sodium extended-release tablets, 75mg, 1-2 times daily orally; or ibuprofen extended-release capsules, 0.3, 1-2 times daily orally. The application of NSAIDs needs to be combined with the application of gastric mucosal protective agents to avoid or reduce the damage to the mucosa of the digestive tract. Use with caution in patients with gastrointestinal diseases or liver and kidney dysfunction. After the disease is in remission, further treatment needs to be decided according to the blood uric acid value and the condition. It also needs to be combined with dietary adjustment, regular review, keeping away from triggering factors and proper exercise.
