What is the basis for determining psychosexual disorders?

   Psychosexual disorder used to be called sexual perversion or sexual inversion, refers to a group of mental disorders in which the sexual psychology and sexual behavior of both sexes deviate significantly from normal, and the deviant behavior is the main or only way of sexual excitement and satisfaction. The normal sexual activity is totally or somewhat disrupted, disturbed or affected. There is no other significant abnormality in the general mental activity.  The exact prevalence of psychosexual disorders is difficult to estimate.  The International Classification of Diseases, 10th edition (ICD-10) lists three main categories of psychosexual disorders: sexual identity disorder, sexual preference disorder, and psychological and behavioral disorders related to sexual development and sexual orientation.  In 1973, the American Psychiatric Association Council was convinced that homosexuality was not a mental disorder. In removing homosexuality from the Diagnostic and Statistical Manual (DSM) as a category of mental disorder, the society declared that “homosexuality does not in itself imply impairment of judgment, stability, trustworthiness, or general social or occupational competence ” (American Psychiatric Association, 1973). However, the revised manual (DSM-3) included the treatable disorder unit of “self-incongruent homosexuality. In subsequent revisions, the DSM-3-R list of disorders did not include either homosexuality or self-incongruent homosexuality.  In the DSM-5, “sexual identity disorder” and related clinical phenomena were removed from the psychosexual disorders and listed as a separate category of mental disorders called “gender anxiety disorder (gender dysphoria).  As a result, the DSM-5 classification of psychosexual disorders includes the following common types: voyeurism, exhibitionism, fetishism, sexual masochism, sexual sadism, pedophilia, fetishism, transvestism, and others.  Sex is a basic human need. Through the biological, psychological and social functions of sexuality in both sexes, humans have achieved racial and social stability and development.  Human sex has three functions: biological function, that is, reproductive function; life function, that is, the sexual experience is an important life experience, it is an important role in promoting personal growth, is an important aspect or content of life, can bring people psychological pleasure and satisfaction, reflecting some meaning and value of life; social function, that is, the function of establishing and developing interpersonal relationships, Chinese culture emphasizes that husband and wife are Chinese culture emphasizes that husband and wife are the first of human relationships. Sound sexual psychology and sexual behavior should be appropriate and harmonious to complete the above three functions. People’s sexual psychology and sexual behavior are highly variable, therefore, mere statistical deviations cannot be considered as psychosexual disorders. In clinical psychiatric practice, the definition or diagnostic criteria of psychosexual disorders cannot be separated from the influence of society and culture; different societies and different cultural backgrounds have different evaluations of sexual behavior. Even the same society and the same cultural background may have different evaluation criteria at different times. Therefore, there is no absolute standard for measuring normal or abnormal sexual behavior so far. The distinction can only be conditional and relative. Initially, many sexual behaviors were diagnosed as “sexual deviance” and were medically stigmatized. Later, sexual behavior was considered normal for the majority and abnormal for the minority, including reproductive purposes and sexual intercourse between men and women, and homosexuality was considered a “sexual minority”.  Nowadays, the view of sexual pluralism is that sexuality is equal and exists in pluralism. This view is based on the values that all people are equal, that men and women are equal, that sexuality is a human right, and that “the state respects and guarantees human rights.  Modern society believes that each individual, is unique, including in the way they behave sexually; there are differences in sexual behavior between individuals. Modern society is becoming more and more open in terms of sexuality, respecting individual sexual rights and individual differences in sexual activities, believing that there has never been a uniform standard in human sexual activities, but rather promoting pluralistic sexual values to defend the dignity and value of each individual’s unique sexuality. This is called “sexual pluralism”. Differences are not superior or inferior. In sexual behavior, there are differences between individuals, and by emphasizing respect for differences, we can truly achieve equality between men and women.  Sexuality is a human right. Every person has the right to decide how to use his or her own body, including sexual organs. If a uniform standard is set for sexual behavior, it is tantamount to “sexual moral hegemony,” which means: I am right, you are wrong; those who have the right are right, those who do not have the right are wrong; the majority is right, the minority is wrong; reproductive is right, non-reproductive is wrong; those who have love are right, those who do not have love are wrong; and so on. None of these are consistent with sexual human rights.  Sexual freedom is not sexual promiscuity; sexual freedom is the right of the individual to decide his or her own sexuality. Sex is a private matter. According to Li Yinhe, as long as the three criteria of “adulthood, consent and secrecy” are met, sexual behavior is normal.  With the development and progress of society and culture, it affects people’s attitude and value judgment towards sex, and affects the diagnosis and classification of psychosexual disorders.
