Exploring the clinical application of Peach Kernel Cheng Qi Tang from the theory of opening and closing pivots

Peach Kernel Cheng Qi Tang is derived from the Treatise on Typhoid Fever. The original formula is used to treat the evidence of blood stagnation in the lower jiao of the lower jiao of typhoid fever when the evil in the sun is not resolved and enters the internal organs through the meridians and turns into heat, which combats with blood. This formula is a representative formula for breaking Blood and expelling Blood stasis. The clinical application of this formula has been developed in later times, and it is concluded that the following clinical indications are present: 1. urgent knots in the abdomen and constipation; 2. frenzied or feverish, irritable and sleepless, red face, dry mouth and red eyes; 3. amenorrhea or menstruation with purple and dark clots; 4. red and purple tongue, petechiae and petechiae, sunken or stringy or sunken pulse, etc., then Peach Kernel Cheng Qi Tang can be applied. It is widely used clinically in the treatment of gynecological diseases with blood stasis, intestinal diseases, urinary tract diseases and mental diseases. However, clinical application of Peach Kernel Cheng Qi Tang only from the perspective of the corresponding formula evidence limits the scope of application of this formula. To apply this formula skillfully and expand its indications, it is necessary to master the “six meridians identification” system of the Treatise on Typhoid Fever. The key to discussing the essence of the six meridians lies in the understanding of the “three yin and three yang”. The Book of History and Calendars says: “Even on the day of the Son when the winter solstice is reached, the path of Yin and Yang will be in motion.” This means that the division of the three yin and three yang is based on the changes of yin and yang during the year, and expresses the six states of the separation of yin and yang in nature. Su Wen – Theory of Yin-Yang Reunion says: “The three yang’s reunion also, the sun is open, Yang Ming is closed, Shaoyang is pivotal.” The sun in the northeast, after the winter solstice, Yang gradually open, so the Yang of “open”; Yang Ming in the northwest, Yang gradually closed, hidden in the Yin, so the Yang of “closed”; Shao Yang in the southeast, the summer solstice, the sun returns, Yin and Yang pivot with this, so the Yang of “pivot”. Professor Gu Shishan marked the spatio-temporal orientation of the three yin and three yang openings, closings and pivots on the taiji diagram, forming a taiji diagram of the three yin and three yang (Figure 1 and 2), which graphically expresses the spatio-temporal orientation of the “six qi”, and also graphically explains the respective properties and different characteristics of the “six meridians”. different characteristics. Therefore, understanding the “six meridians” in terms of different spatial and temporal orientations of yin and yang qi, and combining the characteristics of each of the “six meridians” with their prescriptions, will open up better ideas for the application of clinical prescriptions. The patient began to suffer from insomnia 2 years ago, with difficulty in sleeping, accompanied by distress, aggravated in the afternoon and at night, with about 4 hours of sleep at night, and gradually developed facial discoloration, mainly in the two cheekbones, with an atrophic complexion, abdominal pain and fear of cold before menstruation, low menstrual flow, with blood clots, fear of cold in the knees and cold hands and feet, with normal appetite, unformed stool, once a day, normal urine, dark tongue with petechiae on the side, red tongue tip, thin yellow fur, and string pulse. Differentiation: Blood accumulation in the lower jiao and unfavorable pivot. Prescription: Peach Kernel Cheng Qi Tang with Addition and Reduction. Peach kernel 9g Gui Zhi 9g Rhubarb 9g Citrus Aurantium 9g Dried ginger 9g Roasted licorice 6g 4 doses, decoction in water, one dose per day, divided into two doses on an empty stomach. Second Diagnosis: Significant improvement in sleep after the medicine, sleep well, warm hands and feet, heartburn reduced, physical strength, stool twice a day, unshaped, tongue dark to light, tongue red, moss thin yellow, pulse string. Peach kernel 9g, Gui Zhi 9g, Rhubarb 9g, Citrus Aurantium 9g, Ginger 9g, Roasted licorice 6g, Radix Bupleurum 12g, Radix Paeoniae Alba 15g, 4 doses, decoction in water, one dose daily, divided into two doses on an empty stomach. At the third consultation, all the symptoms improved, and Peach Kernel Cheng Qi Tang was added to soothe the liver, invigorate the spleen, and activate blood circulation to regulate the posterior. Analysis: The patient’s clinical manifestations are characterized by: 1. symptoms of blood accumulation in the lower jiao; 2. symptoms mostly aggravated in the afternoon and evening; 3. irritability due to failure to lower heart fire and cold evidence of lower jiao due to failure to warm kidney water. From this, we can see that this patient’s blood is stored in the lower jiao, resulting in a disconnection between yin and yang qi, and yangming cannot hide and close yin. The Four Sacred Sources of the Heart – The Interpretation of the Heavenly Man says: The stomach is turned to the right, and the government of astringency is carried out, so it cools and turns into Xin Jin. Half Yin, Yin is born, Yin is descending, three Yin right down, then for the lung gold, lung gold that is the heart fire of the clear descending also. If Yang Ming dry gold does not descend, heart fire cannot go down to warm the kidneys, which is characterized by irritability, irritability, difficulty in sleeping, aggravated in the afternoon and the first half of the night, and in the lower part of the body, Yang Qi cannot go down to warm the kidneys, and the kidney water is not warm and the lower limbs are cold and the abdomen is cold. In this formula, peach kernel and rhubarb are the main herbs. Peach kernel activates blood circulation and resolves blood stasis; rhubarb attacks and expels blood stasis and directs heat downward; mangiferous softens and resolves blood stasis, and diuresis moves heat downward; cinnamon branch warms the meridians and smooths blood flow, and roasted licorice harmonizes all the herbs. The whole formula invigorates blood circulation, resolves blood stasis, and relieves heat. If the stagnant heat is relieved, yin and yang will be connected, heart fire will be lowered, kidney water will be warmed, and all evidence will be removed. The patient’s facial pigmentation is due to long-term fire-heat decoction and phlegm and stagnation on the upper part of the body, so she should continue to regulate afterwards. Therefore, it can be considered that Peach Kernel Cheng Qi Tang can be used to treat the symptoms of Blood Storage with unfavorable pivot, non-connection of Yin and Yang Qi, and Yang Ming not closing. The clinical application of Peach Kernel Cheng Qi Tang is analyzed by applying the theory of the three yang and three yang pivots to provide a reference basis for broadening its application.
