Each attack is a fever, mostly high, with a temperature of more than 39 degrees, a red and swollen throat, and enlarged tonsils. Almost every time, once a month, it was very easy to get medical attention because my father was a doctor at the town hospital. In the last six months, the attacks were worse than before, almost always with suppuration of the tonsils, and the interval between attacks was significantly shorter, more than 20 days on one, and finally the interval between these two attacks was only half a month. The attacks are more frequent than before, but the hanging water is not as effective as before, and it always takes 5-8 days to get better. The father was also worried about why the child had such frequent attacks, and all the investigations were done, but nothing was found. This is definitely not the way to solve the problem by hanging water for a long time. Don’t look at the child who is always sick, but this child is developing really well, the 6-year-old child has already reached the weight of 8 years old. The family knows that they should let the child eat more vegetables, but since the child is always sick, they can only give in and do whatever they want to eat, hoping that their son will eat more and get better. This child’s stool is usually a bit dry, the front part is coarse, the back part is sometimes dry, sometimes not so dry. He has a little difficulty passing stool, but he can always pass stool once a day or two. Usually the lips are a little redder than other children’s, and usually it is easier to get on fire. Whenever the stool is a little dry and the lips are red, mouth sores will easily appear, and he will easily get fever. Always cold, yellow snot Gogo is a 4-year-old girl, and in the past six months, she always had yellow snot, which started to flow again within a few days after she got better. The doctor went to see her and said it was sinusitis. She often took some antibiotics, but sometimes they were effective, but many times they were not too effective. Because of the repeated attacks, half a month to once a month, parents are also very helpless. Recently, the child has switched to Chinese medicine to clear the heat, and it has some effect, but it is not good enough. The child usually eats well, sleeps well, and doesn’t have many colds, but the stool is a little dry, sometimes like goat poop, one by one, sometimes just a little thicker, but the back is fine. The parents found that the child’s onset was very regular, as soon as the stool became very dry, there would be bad breath, then poor nausea, followed by yellow runny nose. Is it possible that the yellow runny nose is also related to dry stools? The lung and the large intestine are on top of each other, and if there is heat in the large intestine, it is easy to move heat to the lung. In Chinese medicine, there is the doctrine of organism that the human body is a unified body with five organs and six bowels as the core. The five organs are heart, liver, spleen, lungs and kidneys, and the six internal organs are gall bladder, stomach, large intestine, small intestine, bladder and triple jiao. There is a close connection between the five internal organs and the six internal organs, and each pair of internal organs has a close connection in function. When we explain the above case, we often say that “the lung and large intestine are close to each other”, what does this mean? That is to say, the lung and large intestine is the relationship between the surface and the inside, which is the pairing relationship we just said. This pairing relationship determines that each organ pathology may affect the corresponding internal organs, or each internal organ pathology may affect the corresponding organs. In other words, heat in the lungs can move to the large intestine. If there is heat in the large intestine, it can also move to the lungs. For example, some children have dry stools when they have a cold. For example, in these two children, when the stool is dry, they will have a red and swollen throat or a yellow runny nose. In other children, once the anus is red, they tend to have coughing and wheezing, and all these are clinical expressions of the lung and large intestine being on top of each other. The elimination of accumulation and laxative means the removal of heat from the lung. Since heat in the large intestine can be transferred to the lung, it is easy to have a red and swollen pharynx, swollen tonsils, or even pus. So what should be done in this case? The root cause of lung fever is related to the large intestine, and it is clear that the large intestine is the one with heat. Then this treatment should be the bottom of the barrel, clearing the large intestine is imperative. The laxative can relieve the fire, and the laxative is the lung heat. Therefore, the treatment of such patients must not take the stool problem lightly. In the later stage of treatment, we should also pay attention to the stool problem. In the above two cases, the patients had different degrees of food accumulation and constipation. In pediatrics, constipation induced by food accumulation is still relatively common. We know that the cause of food accumulation is eating too much, and when food accumulation does not dissolve, it leads to Qi stagnation, that is, Qi stagnation does not work. Because the Qi does not go down, it may affect the timely discharge of stool and delay defecation time. In addition, food accumulation does not transform, easy to depression and heat, heat in the intestines, it is easy to dry stools. Once constipation is formed, it will also affect the emptying of the stomach and intestines, making the original poor appetite become worse and aggravating the symptoms of food accumulation. In other words, food accumulation and constipation can be the cause and effect of each other. Therefore, when regulating constipation, we must pay attention to the treatment of food accumulation. Preventing food accumulation may, from a certain point of view, prevent constipation. By preventing constipation, it helps to maintain the stability of the internal environment, thus reducing disease. Lung heat is the symptom and internal heat is the root cause In the first two cases, one is purulent tonsillitis and the other is sinusitis, which are two different diseases altogether. From the TCM point of view, both are heat-related diseases. From the Western medicine point of view, both diseases come from infections. From the TCM point of view, both diseases have internal heat as an intrinsic factor. The treatments currently used for both diseases are treating the symptoms but not the root cause, which is the underlying factor for the recurring illnesses in these two children. Of course, treating the symptoms is necessary at the onset of the disease, but this does not improve the constitution and does not prevent recurrence. Therefore, the fundamental way to treat these diseases is to change the existing constitution and bring it into balance with yin and yang. Once the internal environment is stabilized, future morbidity will be reduced. As for how to clear the internal heat, in the case of these two children, the internal heat is manifested by constipation and easy fire, but the formation factor of internal heat may still be internal heat from food accumulation. Therefore, the later treatment should be both laxative and food stagnation elimination, and of course, parents should be guided to feed the children reasonably.
