A 54-year-old man with a history of drinking alcohol for about 30 years was found to have “liver cancer” after consultation!

(Disclaimer: This article is for general use only, and the information in the following content has been processed to protect patient privacy)
Abstract: The patient reported that he had been drinking alcohol for a long time and visited the hospital with pain and discomfort in the right upper abdomen for half a month, accompanied by loss of appetite and wasting. The patient was admitted to the hospital and completed an intensive CT examination of the abdomen, which suggested that a tumor in the right posterior lobe of the liver, with a diameter of about 4 cm, was considered to be hepatocellular carcinoma, a type of liver tumor. The patient underwent hepatectomy under general anesthesia, and the postoperative pathology confirmed that it was hepatocellular hepatocellular carcinoma. Intraoperatively, the liver showed alcoholic cirrhosis, indicating that the patient’s liver cancer was related to long-term heavy alcohol consumption, and the postoperative recovery was smooth.
Basic information】Male, 54 years old
Disease Type】Hepatocellular carcinoma
Hospital】Liaocheng People’s Hospital
Date of consultation】November 2021
Treatment plan】Surgical treatment (hepatocellular carcinoma resection) + drug treatment (sodium penicillin for injection, diammonium glycyrrhetinate enteric capsule, ursodeoxycholic acid tablets, fatty lactic acid (18) injection)
Treatment period】2 weeks of inpatient treatment
Treatment effect] Radical resection of tumor, disappearance of right upper abdominal discomfort
I. Initial consultation
The patient had pain and discomfort in the right upper abdomen for half a month, accompanied by loss of appetite and weight loss, and had lost about 10 pounds in the past 2-3 months. After asking the patient about his medical history, the patient usually had the habit of drinking a lot of alcohol for a long time, at least half a kilogram of liquor every day for 30 years, and in this case, the patient usually had alcoholic cirrhosis and might even have liver tumor, so the patient had liver function tests and abdominal ultrasound. The patient was admitted to the hospital after explaining his condition.
Treatment process
After admission, the patient was further examined and AFP was found to be significantly higher than 3000, which was also highly suspected to be primary liver cancer. The patient’s right liver was found to have a single tumor with a diameter of about 4 cm, and no clear tumor was found in other liver. After explaining the patient’s condition to his family, the patient’s family also agreed to the surgical plan. The hepatocellular carcinoma was resected under general anesthesia and the patient was found to have alcoholic cirrhosis in the liver during surgery.
III. Treatment effect
The patient’s surgery was done very carefully, and the tumor was removed 1 cm outside the outer edge of the tumor, and the tumor margin was also guaranteed. The patient recovered very well after the operation. After the operation, the patient was given anti-infection penicillin sodium for injection, hepatoprotective treatment with diammonium glycyrrhizate enteric capsules, bile relief with ursodeoxycholic acid tablets, and intravenous nutrition with fatty lactic amino acid (18) injection, etc. The patient had exhaustion and defecation within 3 days after the operation and started to eat gradually. The discomfort in the right upper abdomen had disappeared by the time of discharge, and the feeding was also better than before, and the patient was successfully discharged on the 10th postoperative day.
IV. Notes
We are glad that the patient’s condition was cured after treatment, but the patient should also pay attention to rest and avoid strenuous activities for 1 week after discharge, because strenuous activities may cause the incision to split. At the same time, it is necessary to increase nutrition to promote the healing of the incision and the recovery of body functions. Avoid getting cold, because the body’s resistance is weak after surgery. After discharge from the hospital, it is necessary to return to the hospital for a review in about 3 weeks, and there may be a need for hospitalization for an intervention to further consolidate the treatment effect. The main purpose is to detect tumor recurrence and treat it in time.
V. Personal insight
Patients who drink a lot of alcohol for a long time will gradually develop alcoholic cirrhosis, and on the basis of alcoholic cirrhosis, it may develop into liver cancer. Liver cancer may also occur without alcoholic cirrhosis, because alcohol consumption is also an important risk factor for liver cancer. Therefore, if there are patients who drink a lot of alcohol for a long time, the first thing you need to do is to strictly abstain from drinking, and at the same time, review liver function, abdominal ultrasound and abdominal CT regularly. If liver tumor is found, it should be treated in time, and surgery is the first choice, which is due to the fact that surgical resection can achieve the effect of radical cure.
