Aplastic anemia (AA) is a bone marrow hematopoietic failure, mainly manifested by low bone marrow hematopoietic function, complete blood cell reduction and anemia, bleeding and infection syndrome. Chronic Aplastic Anemia (CAA) is a hematologic disorder with slow onset and relatively long course, which is very difficult to treat, and most Western doctors choose to treat it with androgens and cyclosporine. Combined with the pathogenesis of AA in modern medicine, we believe that the conventional remedial therapy still has some limitations. Due to the complexity of the pathogenesis and pathological factors of CAA, it is appropriate to adopt a generous treatment of compound method to warm the kidneys and strengthen the spleen, and to support and detoxify the toxin. In this regard, we conducted a preliminary clinical study and used the “Nourishing Blood and Pinging Barrier Soup” composed of 24 Chinese herbs to treat 21 patients with CAA in whom western drugs (cyclosporine and androgens) were ineffective, with one dose daily, decocted twice in the morning and evening, 150 ml per dose. The efficacy was determined after 2-3 courses of treatment. The follow-up period is 1 year. Clinical efficacy In terms of the efficacy of Western medicine, 3 cases (14.3%) were basically cured, 3 cases (14.3%) were in remission, and 5 cases (23.8%) were significantly improved. There were 10 invalid cases (47.6%), and the total effective rate was 52.4%. In terms of the efficacy of TCM symptoms, 4 cases (19%) were basically cured, 6 cases (28.6%) were apparently effective, 5 cases (23.8%) were effective, and 6 cases (28.6%) were ineffective, with a total effective rate of 71.4%. After treatment, all 21 patients had a significant increase in the blood picture of the three systems, and the difference was statistically significant when compared with that before treatment (P<0.01)< span="">Adverse reactions Individual patients experienced mild diarrhea or dry mouth during treatment, and after adjusting the dosage and ratio of the corresponding drugs to achieve a balance between yin and yang, the relevant symptoms were relieved. According to Chinese medicine, CAA belongs to the categories of “deficiency labor”, “marrow labor” and “blood evidence”. The cause and mechanism of CAA are considered to be the result of external infection with the six evil spirits, internal injury to the seven emotions, poor diet, and overwork, which damage the five organs of the spleen and kidneys, resulting in deficiency labor. We believe that the cause of re-infection is due to the deficiency of positive qi on the one hand, which cannot resist external evil, and the evil toxin takes advantage of the deficiency to enter, further depleting positive qi and affecting blood production; on the other hand, due to the internal trapping of evil toxin, which burns the blood and interferes with the marrow, resulting in stasis and toxicity, or damages the essence of the successive heaven and depletes the marrow, resulting in the lack of blood production source. As a result, the deficiency becomes poisonous and the poison becomes more deficient, forming a vicious circle. It can be seen that deficiency, toxicity, and stasis are the main pathological factors of the disease, and its pathogenesis is characterized by changes in the original deficiency and the actual symptoms, so it is difficult to seek the whole picture in treatment by simply supplementing deficiency. Considering the long duration of CAA, the complexity of the pathogenesis, the variety of symptoms, and the difficulty in healing, we propose a generous compound formula for the treatment of this disease.
