How to choose Chinese herbal cream for diabetic foot treatment?

  The treatment of wounds generally goes through several periods, and each period is treated somewhat differently. Now we will briefly talk about how wounds that are in the inflammatory phase are treated by Chinese medicine. When we gave a presentation at the forum on the combined treatment of diabetic foot in Beijing, we mentioned the theory of simmering pus and generating muscle, which is a Chinese medicine treatment for diabetic foot.  Simmering pus to produce muscle refers to the application of Chinese herbal ointment (powder) on the sore surface to increase pus oozing out of the wound and to carry the evil out, thus promoting the growth of the wound. It was first described in the book “Surgery Qixuan” written by Shen Duoguan, a famous doctor in the Ming Dynasty, in which a very detailed classification of wounds was given, and diabetic foot was only one of them.  But simmering pus and generating muscle is a big theory, but there are still many considerations for specific treatment, such as the use of Chinese herbal medicine depending on the different periods of wound trauma, the type of herbal medicine chosen is also different.  I. Necrotic tissue period. Dry gangrene wounds are often covered with more black, dry necrotic tissue or crust, one of the main purposes of Chinese medicine treatment in this case is to make the local sore surface pus secretion increase, dry necrotic tissue or crust soften, dissolve, fall off, and promote the exposure of the base of the sore surface, in preparation for nibbling debridement. Wet gangrene then there is no softening this step.  Second, the inflammatory response period. The trabecular base is less necrotic tissue, inflammatory exudate is dominant, the trabecular base tissue is obviously edematous, yellow “rotting flesh”, or a small amount of old granulation tissue. At this time can be combined with the use of traditional Chinese medicine oil combined with massage, ointment wet compress, one of the main purposes is to rebuild microcirculation, improve the ability to fight infection.  Third, granulation proliferation period. At this time, the base of the wound is fresh and red, the granulation tissue proliferates and fills the wound defect, and the epithelium of the wound edge begins to proliferate and crawl or form “skin islands”. The main purpose of herbal medicine at this time is to improve the proliferation ability of granulation tissue and promote the rapid growth of granulation.  Fourth, the epithelialization period. In this period, the granulation tissue basically fills the base of the wound, and the purpose of treatment is to promote the proliferation of epithelium to form healthy skin and eventually achieve wound healing.  However, the basis of TCM treatment still needs to be combined with Western medicine, because diabetic foot patients are mostly middle-aged and elderly, and they have poor health conditions, blood sugar, blood pressure, blood lipids, cardiovascular and cerebrovascular, metabolic disorders, infections, etc. All these need Western medicine treatment. Therefore, combining Chinese medicine and Western medicine together, going to the length of both, the treatment effect will be better instead, and this is one of the main reasons why we use the combination of Chinese and Western medicine.  But in any case, the treatment of wounds by TCM is irreplaceable by Western medicine.
