The patient, a 31-year-old male, presented to the clinic with swelling and pain after penile jackknifing and fracture mainly due to drunken intercourse. On examination: the penis was swollen, with marked dorsal bulging and painful to palpation, without hematuria. Ultrasonography using a high-frequency probe: the continuity of the dorsal cavernous body was interrupted, the broken ends were separated in a barbed pattern, and irregular liquid dark areas were seen between the two broken ends, within which fine dotted echogenicity was seen. Ultrasound diagnosis: cavernous body disruption and hematoma formation. Discussion: The penile corpus cavernosum is a thin cylinder with two pointed ends, one on each side, located on the dorsal side of the penis. The right and left ones extend forward in close union, and the anterior end becomes thin and embedded in a depression at the base of the penile head. The posterior end of the corpus cavernosum separates and is called the pedicle of the penis, which is attached to the subpubic and sciatic branches on each side. The interior of the corpus cavernosum consists of many cavernous trabeculae and cavities, the cavity is a sinus gap connected with blood vessels, the cavity is filled with blood during erection, the trabeculae are stretched and thin, the penis becomes thicker, harder, and easy to fracture by bumping. The spongy intercourse rupture, mostly due to sexual intercourse is too reckless, force, penile head against the female pubic symphysis, the penis was hit by the fracture, resulting in blood-filled spongy body rupture. Ultrasonography can clearly probe the continuity of the cavernous body interrupted, the broken end is barbed, there is hypoechoic hematoma formation between the two ends, combined with the medical history, ultrasound diagnosis is basically correct. Although there are reported cases of spongiosomal rupture diagnosed by ultrasound due to sexual intercourse, the number of cases is small and rare, so it is reported again.
