With the deepening research on the etiology and pathogenesis of Chinese medicine, the role of “toxic evil” in the pathogenesis and transformation of diseases has received more and more attention. In the study of many major diseases, such as cerebrovascular disease, cardiovascular disease, diabetes, kidney disease, tumor, AIDS, systemic lupus erythematosus, rheumatoid arthritis, liver disease and infectious diseases, the view that “toxic evil” causes disease has been constantly mentioned and strengthened. From Angong Niuhuang Pill to Qingkailin, to further research on the small compound of the components of antipyretic drugs, we have completed the verification process from drugs to disease mechanism, and then from disease mechanism back to prescriptions, and the disease mechanism and efficacy have been verified. More levels of basic experiments [35] and more perspectives of clinical studies supported and verified the view that the pathogenesis of stroke disease is poisoned and damages brain ligaments. Toxic evil originates from all evils, no matter phlegm, stasis, wind or fire, once brewed into poison, it can still reflect the characteristics of the original disease evil, but its pathogenic effects are all more than the original disease evil ever. The inflammatory response theory of AD has been proposed in recent years, which has further prompted us to re-examine the TCM etiology and pathogenesis of AD. The series of inflammatory changes in the process of AD, such as increased levels of inflammatory response markers and inflammatory mediators, should be related to “toxicity and stasis” in traditional Chinese medicine. Many researchers have explored the relationship between internal toxicity, heat injury to the brain and Alzheimer’s disease in theory and practice. From the holistic viewpoint of Chinese medicine, when the internal organs are weakened or attacked by external evil, the original balance of the body is broken, resulting in the dysfunction of the internal organs of the human body, the malfunction of qi, blood and fluid, the physiological and pathological products in the body, such as phlegm and blood stasis, cannot be eliminated in time, and these internal toxicity accumulates in the body. These endogenous toxins accumulate in the body and turn into heat over time to form “turbid poison”. After the formation of endogenous “turbid toxin”, it corrupts the body, consumes the internal organs and meridians, and eventually leads to “turbid toxin” stagnation in the ligaments and veins, so that the qi and blood cannot be spread and infused, therefore, Chinese medicine has the saying of “long-term disease into the ligaments “When turbid toxins damage the brain ligaments, the brain orifices will become congested and dementia will occur when the mental mechanism is not unified. Therefore, based on the doctrine of inflammatory response in AD and the doctrine of internal toxins causing disease in TCM, we believe that AD caused by toxins should be supplemented, and the treatment of clearing heat and detoxifying toxins should be one of the treatment rules for AD. This can further improve the TCM treatment rules of AD, and help to improve the prescription of “Brain Returning Dan” which we have been studying for many years, so as to develop more effective drugs for the treatment of AD. As the treatment methods and rules of TCM have to be realized by the corresponding traditional Chinese medicine, we propose to use the classical formula of Huang Lian Detoxification Tang for clearing heat and detoxifying toxins. Huanglian Detoxification Tang is composed of Huanglian, Scutellaria, Huangbai and Gardenia (Waidai Secrets), the whole formula has the effect of clearing heat and detoxifying and diarrhea of fire, mainly treating the evidence of heat in the three jiao and fire-heat and toxicity, which is the classic representative formula of clearing heat and detoxifying.
