Can TCM see difficult and critical illnesses?

Bacterial infectious diseases are now extremely common in clinical practice, and the invention of antibiotics has enabled many patients to have a chance to save their lives. However, due to the long-term clinical use of antibiotics, the clinical efficacy of many antibiotics has been greatly reduced due to the increasing number of drug-resistant bacterial beads, and many infections cannot be effectively controlled. At the same time, antibiotics are like a double-edged sword, sometimes in the “cure” at the same time, but also “cause disease”, some powerful antibiotics such as Tylenol, Mepin and so on, often induced fungal infections, intestinal flora dysbiosis. In contrast, Chinese medicine has good advantages in anti-infection, and reasonable and early use of Chinese medicine interventions can greatly improve clinical efficacy and even give new life to dying patients. In the current public mind, TCM is the best choice for chronic disease treatment and health care, and it is believed that acute diseases should be treated by Western medicine first, because Western medicine has advanced treatment methods, convenient drug delivery methods, and drug efficacy comes very fast, while the effect of TCM is slow, and it is a wise choice to choose Western medicine for treatment of serious and major diseases. It is true that Western medicine has its advantages in many aspects, but it is not a wise decision in many cases to put all our hopes on Western medicine for critical and serious illnesses, which will make us lose the hope of healing and life extension. 1. subarachnoid hemorrhage coma 37 days combined with severe pulmonary infection Let’s start with a case of a female, 70 years old, with previous history of hypertension and diabetes mellitus. This patient was the mother of a director of a provincial inspection hospital, a patient with subarachnoid hemorrhage, who had an onset on November 4, 2007, and was sent to a medical school for surgical treatment in the Department of Neurosurgery the day after the onset. According to the family, before entering the operating room, the person was conscious, and when he came out of the operating room, he was unconscious (all extracerebral surgeries may be like this, not related to the doctor), and developed a serious lung infection, and had a tracheotomy. Western medicine is a variety of high-grade antibiotics, the condition did not improve significantly, the doctor recommended the transfer from the care unit to the general ward, the general ward doctors are not willing to accept, why? This patient is a “hot potato”, the lung infection is not allowed to aggravate, and the patient may not wake up for a year or two, so it is recommended to transfer to their Chinese medicine department. So the family asked the surgeon in charge of the surgery, a famous specialist in Hunan. The specialist said that a combination of Chinese and Western medicine might be better and suggested not to stay in their hospital. He said, “The TCM doctors in our hospital are not real TCM doctors, they are guiding the use of TCM according to the Western medicine pharmacology, go to an authentic TCM hospital! Go to the Provincial Hospital of Traditional Chinese Medicine, which is the rocking blue of TCM in Hunan Province. Almost all of the major Western medical experts I have encountered are very much in agreement that true traditional Chinese medicine is not the view that is circulating in society. This patient’s family then came to contact me for communication and after explaining the various risks, the patient understood and said that any problems will not find you in trouble, huh, I said, then you will turn over. When the patient was transferred to me on December 6, it was already more than 30 days after the operation. When he was admitted to the hospital, he was in a waking coma, and he said that his eyes were sometimes wide open, but he did not respond to external stimuli, his right limb was hemiplegic, his teeth were closed, his throat was full of sputum, and white sputum was constantly pouring out from the tracheal tube, and the nurse kept inserting sputum saliva outward. There was a lot of sputum sound in both lungs. The companion said that occasionally when he opened his mouth, he saw his tongue was red and his moss was yellow and greasy. His pulse was smooth. The family said that there were occasional paroxysmal convulsions, which were determined to be secondary to epilepsy based on previous medications. The patient was in a really bad condition, with tracheal intubation, nasal feeding tube, urinary catheter, oxygen tube, and infusion tube, heck, they were all there, none of them missing. The treatment of western medicine prescribed a long medical advice, but only nerve nutrition, energy balance water electrolytes ……, are some routine, can be said to be the same as the patient in the medical school hospital treatment, there is no special approach to this patient western medicine, to put it bluntly is symptomatic treatment. The patient’s lung infection is very heavy, antibiotics can not be used. However, the doctor was concerned about the choice of antibiotics and discussed with me which one to choose, why? The antibiotics we have are all used in western hospitals, and many patients have used high-grade imported antibiotics we simply do not have, according to the family, on antibiotics are thousands of dollars a day or even thousands of dollars. The only one available is a fourth-generation cephalosporin – cefpodoxime, it is high. In fact, this drug patients have long been used. The hope of fighting the infection is not put entirely on the Western medicine. The Chinese medicine according to the identification, evidence of phlegm and heat in the lungs, obscuring the heart and soul, treatment when the heat to cleanse phlegm, awaken the spirit to open the tricky, the formula to clean phlegm soup with calamus Yujin Tang plus reduction. The prescription is as follows: Citrus aurantium 10, Bamboo rhizome 10, Chen Pi 10, Jiang Hanxia 10, Yun Ling 15, Acorus calamus 10, Yu Jin 10, Yuan Zhi 10, Platycodon grandiflorus 10, Qian Hu 10, Tian San Qi 3 (punch), Astragalus membranaceus 30, Astragalus membranaceus 10, Atractylodes macrocephala 10, December 10, room inspection, the companion complained of daily diarrhea watery stool 1 to 2 times / day, phlegm significantly reduced compared to the previous, the patient opened his mouth to yawn, see his tongue light white, white greasy coating. The pulse was still slippery. The prescription was changed as follows: Astragalus membranaceus 30, Radix et Rhizoma ginseng 5, Atractylodes macrocephala 10, Acorus calamus 10, Lentil 10, Chen Pi 10, Radix et Rhizoma ginger 10, Fructus lucidum 10, Panax notoginseng powder 3, Radix et Rhizoma radix 15, Radix et Rhizoma angelicae 20, Radix et Rhizoma boneset 10, Radix et Rhizoma boneset 10. The rales in both lungs have decreased significantly, and he still has diarrhea. The above prescription was followed by 5 doses on the 22nd. After admission to the hospital, the patient’s lung dye was basically controlled by this combined Chinese and Western medicine program for more than 10 days, and the two lungs basically did not hear much rales anymore. No convulsions occurred once. However, he still had diarrhea, 5 to 6 times a day. This is a very good effect for this kind of patient. However, the patient’s family disagreed about whether to continue using Chinese medicine. Why? Before the patient was transferred to our hospital, he had loose stools only 1 to 2 times a day, but after he came to our hospital for treatment, the number of stools increased to 5 to 6 times a day. What should I do? In my opinion, the sub-patient’s diarrhea was caused by dysbiosis with antibiotics, and I talked to the patient’s family, who did not quite agree with this opinion. Since the lung infection has been significantly controlled, the patient’s airway is open, it is difficult to have no sputum at all, although there is still the possibility of aggravating the infection, at this time we made a bold decision. We discontinued the antibiotics and added Jin Shuangqi and Emmenthal. The diarrhea stopped on December 28, but there was no significant change in other conditions. The family agreed that the flora was out of balance. Agreed to continue the use of Chinese medicine. Drowsiness, indifferent to external reaction, sputum is still aspirated from the intubation tube, the tongue is pale and fat with teeth marks on the side, the moss is thin and white, the pulse is sunken and thin. The “Treatise on Typhoid” says “Shaoyin is a disease, with a sunken and thin pulse, but the desire to sleep”, coupled with a pale and fat tongue with teeth marks on the side, considering that there is a deficiency of Shaoyin Yang Qi. The sutra says “Yang Qi is like the sky and the sun, if it loses its place, it will break the life and not manifest”, “Yang Qi is the essence that nourishes the spirit”, therefore, the formula for strengthening the spleen, resolving phlegm and opening the orifice is supplemented with sliced sapodilla and cinnamon stick. The patient was able to lift his left hand at the request of his family and doctor, and could open his mouth on his own, and could contract his right lower limb on his own at his request, and there was no obvious change in his tongue and pulse. The patient had a small amount of bowel movement when the patient closed the air during the suction of sputum via tracheal intubation, and no diarrhea at other times. His mental state further improved, and his family complained that the patient gave vague answers to his daughter’s questions (tracheal intubation was not dialed out), but he could understand his meaning, his answers were basically correct, and the muscle strength of his right upper limb improved and he could make a half fist. The upper prescription was supplemented with pomegranate peel 10 and red stone finger 10. On January 6, he was re-examined: he could communicate with the doctor when the diarrhea stopped, but his speech was slurred. The previous prescription was re-introduced. January 12: the patient has been able to chew and swallow on his own for 3 days, the examination has been able to cooperate, the answer to the tangential questions, responsive, has been reliable up to this point, the first stage of treatment can be said to be very satisfactory. Or, after the surgery, it takes some time for the mental recovery, or it depends on the patient to recover by himself, but I think the treatment of Chinese medicine still played a certain role in promoting. Throughout the treatment, a combination of treatment aided by playing music and family call communication (whether the patient was conscious or not) was used. The tracheal intubation was removed in 1 month of hospitalization, and the patient’s speech and physical functions recovered very well. At the time of discharge, he could stand on his own and walk with assistance, and his speech and diet were normal. There is only one case of such a serious patient that people may think is coincidental, and it is difficult to say certain problems. So let’s move on to 1 case briefly. 2. basilar artery occlusion syndrome combined with severe pulmonary infection This patient was the president of a branch of a bank on Zhongshan Road in Changsha, male, more than 70 years old, admitted to the neurology department of a medical university hospital for sudden onset of unconsciousness and tetraplegia, diagnosed as basilar artery occlusion syndrome by MRI, MRA and other examinations, and then complicated with treatment of severe pulmonary infection, given a tracheotomy, using Tylenol, Mepin, Vancomycin and other A variety of antibiotics high-grade antibiotics, did not achieve significant results, and complicated by fungal infection. Later, he was treated with pulmonary lavage, but the infection was still uncontrollable. The hospital sputum culture plus drug sensitivity test showed that dozens of antibiotics were all resistant except vancomycin. However, vancomycin had been used for more than 10 days in a row, but the patient’s condition did not improve significantly. A director of the department, who had studied in the United States and was a well-known specialist in neurology in the province, suggested to the family that the patient be transferred to our department for treatment. Before being transferred to our department, he had been resuscitated in the intensive care unit of the hospital for 38 days. When he was admitted to our hospital, his condition was already very critical, and the patient was delirious and a large amount of sputum was constantly gushing out from the tracheal tube. We still used Chinese medicine as the leading treatment method, combining Chinese and Western medicine. Less than 1 week after admission, the lung infection was rapidly and effectively controlled. Through nearly two months of treatment, the patient’s condition was stable, his confusion turned clear, the tracheal tube was dialed out, speech and limb functions were restored, and the patient could get out of bed and sit in a reclining chair for a long time, and his condition was developing for the better. With a severe lung infection, according to common belief, only powerful antibiotics can effectively control the condition, but is this the case? We will continue here with a few cases. 3. intertrochanteric fracture and severe pulmonary infection Yu, female, 89 years old, was admitted to the hospital with intertrochanteric fracture and pulmonary infection, and was treated with a combination of antibiotics such as cephalexin and clindamycin, but her condition was not effectively controlled. The patient was pale red and dry at the base of the tongue, and when the mouth was opened with a tongue depressor, the tongue was pale and fluid-rich, the moss was little and white, and the pulse was sunken and thin. Both lungs were full of dry and wet rales. Cardiac monitoring showed: sinus rhythm, 110 beats/min, oxygen saturation 87%, respiration 30 beats/min. The blood sodium was 126 mmol/L. The condition was very critical. In this case, our department boldly used Chinese medicine treatment while appropriately adjusting antibiotics. Based on the evidence, it was concluded that there was a deficiency of Shao-Yin Yang Qi and cold drinks in the lungs. On October 2, the patient’s children complained that he took the medicine at noon yesterday, and 2 hours later, he coughed up a large amount of white foamy sputum, and there was a sputum pellet. 21:00 hours: the patient’s consciousness became clear, ECG monitoring: sinus rhythm, 93 beats/min, oxygen saturation 98%, respiration 22 times per minute, and the rales in both lungs were greatly reduced. Subsequently, the prescription was adjusted with the evidence and the lung infection was cured in 10 days. A daughter of the patient, a nurse from Tongji Medical University, said with emotion after her mother’s illness was controlled, “I have been working in the hospital for more than 30 years, but I always had misunderstandings about TCM, and you have shown me the true efficacy of TCM, which is amazing! . 3. Bronchial dilatation and severe pulmonary infection This is a 38-year-old female case with a history of bronchial dilatation. Before admission, she had been hospitalized for more than 3 months in a hospital attached to a medical university in our province and a municipal hospital in Changsha due to pulmonary infection. Later, a friend suggested that western medicine was not effective and that he could try Chinese medicine treatment instead. So the patient came to our hospital for inpatient treatment. When he was admitted to our department, a large number of dry and wet oral rales could be heard in both lungs, and he kept coughing up a large amount of green thin sputum, and he was extremely tired and weak. After being admitted to our department, the patient refused to be treated with antibiotics and asked for TCM treatment. According to the evidence, we used the ancient formula Ling Gan Wu Wei Jiang Xin Tang plus and minus, combined with Western medicine intravenous support therapy, only 1 week, the lung infection was effectively controlled, 20 days infection clinical recovery from the hospital. When he was discharged from the hospital, he said, “It’s still good to have something from the old ancestors (Chinese medicine is good), that is, cheap and effective!” There are many other cases of severe infections that have been successfully rescued and treated using TCM as the main treatment in the key specialty of cardiovascular and cerebrovascular medicine in our hospital. Here, we warmly remind patients, when you suffer from infectious diseases, using antibiotics can not effectively control the disease, do not put your hope on antibiotics, should be early to use Chinese medicine therapy, perhaps not only your condition can be rapidly improved, but also can save a lot of treatment costs.
