What is hemangioma in children?

A. Etiology Hemangioma is a congenital malformation of the development of choroidal vessels, which is formed by the proliferation of vascular network during embryonic period. The majority of them occur in the skin and subcutaneous tissue, followed by oral mucosa and muscles, while liver, spleen and bone are rare. Most of the cases are solitary. Body surface to the head and face > trunk > limbs and buttocks. Capillary type hemangioma is formed by the increase or expansion of capillary network in the dermis. They appear as bright red or purplish-red plaques. They are flush with the skin surface or slightly elevated, with clear borders, irregular shapes and different sizes. (1) Neonatal spots: Skin color changes on the head and neck, disappearing within a few months. (2) Strawberry hemangioma: the most common type of hemangioma, with clear boundaries, uneven surface and strawberry shape, entering the stable stage from half to one and a half years old, and can fade away by itself. (3) Wine spot: capillary network expansion formation, belongs to the vascular malformation, pressure fading, not higher than the skin, irregular shape, clear boundaries, lifelong non-fading, area increases with skin growth. 2.Cavernous hemangioma consists of enlarged vascular cavity and blood sinusoid lined with endothelial cells. Some of them have clear boundaries with intact envelope, while some have unclear boundaries. In recent years, it is believed that some of them are tumors and some of them are vascular malformations, which rarely fade away on their own (5-50%). 3.Tracheal hemangioma is formed by the traffic between small arteries and small veins which are extremely dilated. There is a feeling of worm mass on palpation and tremor, and there is a wind-like murmur on auscultation. It cannot subside on its own. 4.Mixed hemangioma: It is generally considered to be a mixture of strawberry capillary hemangioma and cavernous hemangioma, similar to strawberry capillary hemangioma with a tendency to fade away. Treatment 1. Capillary hemangioma: Because of its tendency to heal by itself, it can be observed at the age of 1-2 years and is mainly applied to children who are not obvious in the receding, stable and proliferating periods. Wine discoloration can be treated by pulsed laser therapy surgery. For proliferative stage giant hemangioma, laser, hormone, interferon, pinyamycin, radiotherapy, medicine, surgery and other treatment methods should be considered according to the doctor’s experience, lesion development, patient condition and other comprehensive considerations. 2.Cavernous hemangioma: injection therapy, electrochemical therapy, and surgery. 3.Tracheal hemangioma: surgical treatment, embolization treatment, compound treatment.
