Determining the nature of thyroid nodules

Patient: On July 30, 2012, a physical examination revealed a cystic nodule of approximately 0.4*0.4 in the right lobe of the thyroid gland. Two isoechoic nodules were seen in the lower pole of the left lobe, the larger one was about 0.7*0.4. Another ultrasound was performed on September 25: the size and shape of both lobes were normal, the envelope of both lobes was intact, the parenchyma of both lobes was not homogeneous, 0.4*0.2 hypoechoic nodules were seen in the right lobe with a little strong echogenicity, 0.4*0.3 hypoechoic nodules could be seen in the right lobe with flush sides, 0.8*0.4 hypoechoic nodules were seen in the left lobe with clear borders. The isthmus was 0.4 in diameter, with a border of 0.4. The diameter of the isthmus was 0.4, and the internal echogenicity was uniform. Several hypoechoic nodules are seen in the neck bilaterally, with uniform borders, the larger one on the left side is 1.6*0.6 and the larger one on the right side is 1.7*0.6 Never treated. I am very worried, please tell me the nature and whether surgery is needed.
