Cirrhosis of the liver as a common disease, a greater danger to human health, but also has its own special clinical features, today’s Chinese medicine should also be recognized as an independent disease, which is conducive to the search for a systematic and more effective treatment. Now, according to my own clinical practice, I would like to talk about the knowledge and treatment experience of this disease. I. Pathological interpretation of common symptoms From the point of view of the clinical manifestations of cirrhosis patients, most of them are in poor nutritional status, sallow color, emaciation, indicating that the body has been weak in positive qi, yin and yang imbalance. The distension and pain in the epigastric region, belching, is gas depression; upset and easy to anger, insomnia and dreaming, mouth sores and tongue rotting, dry ulcer and red ulcer, is fire depression; dullness and generalized evil, chest tightness and epigastric distension, heavy and sleepy, loose ulcer, or ascites, is wet depression; abdominal distension after eating, belching rotting and vectors, bad smell of food odor, thick and greasy tongue coating is food depression and accumulation of stagnation. All of the above manifestations are the result of depression. As for the hardness of the liver on palpation, the pain does not move, the varicose veins of the abdominal wall, the varicose veins of the lower esophagus and the fundus of the stomach, and the history of vomiting blood and black stools, all of them belong to the stagnation of blood circulation. Although there are a lot of clinical witnesses of cirrhosis, there are only three pathological changes: (1) depression, gas, fire, dampness, food depression, which is the key; (2) stasis, stasis of blood agglomeration for the development of all the unresolved depression; (3) deficiency, deficiency of positive qi, yin and yang disorders, which is the result of all the injuries caused by the depression, and dysfunction of the viscera and internal organs. Among the three, “depression” is the fundamental pathology, and the mechanism runs through the whole process of cirrhosis occurrence, development and change. The birth of the disease from this, the disease of even also from this. Second, the commonly used Chinese medicine treatment method synthesizes the relevant information, Chinese medicine on the treatment of liver cirrhosis has the following views. (a) The disease has been prolonged for a long time, the positive qi is already in great deficiency, although the symptoms are numerous, it is difficult to attack from the actual evidence, it is necessary to make up for the positive in order to slow down the figure. According to the different symptoms, or tonifying the spleen, or tonifying the kidney, or nourishing the blood, or benefiting the qi, tonifying the main positive, positive enough since the evil can be retreated. (B) the disease is a standard of this deficiency, this deficiency can be slow to figure, the standard of real must be attacked urgently, with the boat car pills, Gan Sui powder, ten jujube soup and so on the first chase its water, to be the standard of real has gone to support the positive. (3) Emphasizing that blood stasis is the main cause of the disease, advocating the elimination of blood stasis as the main treatment, and using Shimo Yuxuei Tang (下瘀血汤) to treat the disease with rhubarb, peach kernel, Chinese parasitic worms, salvia root, red peony, and turtle shells, and so on. (d) It is believed that in addition to blood stasis, there is also dampness and heat, so it is advocated to activate blood, move qi and clear away dampness and heat at the same time. (5) It is believed that the evidence is complicated and there is no definite rule for treatment, and it is advocated that treatment should be applied according to the evidence: those with dampness-heat accumulation should use the method of clearing dampness-heat, those with weak spleen should use the method of tonifying the middle and benefiting the vital energy, those with liver depression and stagnation should use the method of dispersing liver and regulating the vital energy, such as the method of Chaihu Shuohexuan, those with stasis of blood and obstruction of collaterals should use the method of activating blood and clearing collaterals to stop the pain, and those with impotence, lumbar pain and darkness of the face should tonify the liver and kidney, and use the eight-flavored pills and so on, and those with red eye and thin body, five heart-rending heat and urinating redness with knotting of the urine and stools should be considered as the method of yin deficiency and fire. Some people advocate that when plasma protein decreases, it should be tonifying the middle and benefiting the qi, and reuse atractylodes and so on; some people also advocate to use Drabanemerosa hebecarpa to excrete the lung qi, to use Sanlang and Curcuma to break the blood accumulation, to use Dibaozhi and Cricket to facilitate urination, to alleviate the ascites, and to relieve the symptom. Cirrhosis segmental therapy All the above are commonly used clinical methods, but still lack of systematic and comprehensive. In my opinion, although the symptoms of liver cirrhosis are complicated, as long as we can grasp the three main disease mechanisms of “depression”, “stasis” and “deficiency”, we can distinguish the primary and secondary treatment, and take the main treatment of regulating the liver and regulating the stasis with Activating blood circulation and removing blood stasis, softening and stabilizing with salty cold, adjusting yin and yang, and carrying out treatment step by step and stage by stage, can often receive satisfactory results. Stage 1: dredge the liver and regulate depression. This stage is aimed at dredging and regulating the qi to resolve the depression. The medicines used are Chai Hu, Scutellaria baicalensis, Radix Paeoniae Alba, Cicada Shells, Pieces of turmeric, Almonds, and Jiao Maidenhair. Although regulating depression is the main focus, it must be regulated depending on where the depression is located. Heat and depression should be cleared, fire and depression should be developed, gas and depression should be relaxed, dampness and depression should be opened, food and depression should be eliminated, and stagnation and stagnation should be passed. All the depression is different, how to break it? Must refer to the tongue and pulse to determine where it is. Such as the heat depression, the fire depression, the tongue must be red, the moss must be yellow, the pulse must be stringy and powerful, may add the gardenia, the genus of danpi; The pulse of the gas depression more sinks the string and the stagnation, when the addition of the Buddha’s hand, and so on; The tongue of the wet depression is white and the moss is greasy, the pulse comes to the moistening and softness, it is appropriate to add the patchouli, the genus of the orchids; Food stagnation depression, the tongue moss is thick and the turbidity is greasy, the pulse comes to the slippery solidity, the shutting part is very much, when the addition of the Baohewan, Jiao Sanxian, and so on. Stage 2: Invigorating blood circulation and removing blood stasis. When applying this method, after regulating the depression, the depression is gradually resolved, and then use the products of activating Blood and removing Blood stasis to smooth the Blood flow. However, it is not purely to use products to remove blood stasis, but should be used to regulate qi and relieve liver heat. The gas machine gets smooth then the blood stagnation and line, the liver heat gets drained then the depression does not come back to life also. The medicine uses the danpi, the gardenia, the red white peony, the tulip gold, the almond, the cedar herb, the whirligig flower, the white-headed weng, the jiao sanxian. The third stage: nourishes the blood and activates the blood and softens the hardness and removes the blood stasis. This method is applied after the last two methods. The depression has been resolved, blood and qi are gradually smooth and harmonious, the functions of internal organs are gradually restored, diet and sleep tend to be normal, but only the liver hardness is not eliminated. At this time, it is appropriate to use this method to nourish blood and activate blood circulation, and regulate the qi, and at the same time, use salty and cold to soften hardness and eliminate stagnation of stasis, in order to slow down the elimination of platoon hardness and soften the liver. This is based on Four Substance Tang, which nourishes and invigorates Blood, with the addition of tulip gold, motherwort and cynomolgus to strengthen the effect of invigorating Blood and removing blood stasis. It can also be used clinically with pills such as Tetrapod Decoction Pill. Stage 4: Adjustment of yin and yang for balance. This is the principle of the later stages of treatment. However, the above treatment rules should be applied in conjunction with each other. If Yang is insufficient, support its Yang; if Yin is insufficient, take care of its Yin; if Qi is insufficient, benefit its Qi; if Blood is insufficient, nourish its Blood; if it is weak, replenish it; if it is solid, diarrhea it; if it is cold, heat it. The purpose of the above four stages of treatment is: the first to open its depression, the gas line blood smooth; followed by activating blood circulation and removing blood stasis, salty and cold to soften the nut, blood stagnation is feasible, the liver hard can be soft, the evil go, the positive is also restored. If the hands will be salty to soften the solid or cast tonic medicine, glue to hold the qi, aggravate the depression, and lead to the retention of evil. If ascites is more prevalent or positive deficiency has been extreme, and when the first water to treat the symptoms or urgent to support the positive, often in the changes in the method, noble in the evidence weighing. Fourth, the cirrhosis of life management Clinical treatment of cirrhosis is not ideal, often because of over-reliance on drug therapy, but do not pay attention to life management. Life management should focus on the following aspects. (A) careful diet: food should be light, vegetarian diet, eat less and more meals. Try to eat less fat and high protein food, and eat more fresh vegetables, to maintain smooth bowel movement. The purpose is to reduce the burden on the digestive organs and liver. Clinical often because patients and their families one-sided emphasis on increasing nutrition, forced to eat {nutritional food, resulting in loss of appetite, epigastric distension, abdominal discharge more than, thick and greasy tongue, in order to increase the burden on the gastrointestinal and liver, although the nutrition is high, but there is no absorption, into the leakage of two, the loss is not worth the gain, the food in the middle, but increase the liver damage. (ii) more activities: the purpose is to make the circulation of blood and qi, spit out the old and new, in order to facilitate the recovery. Liver patients are often easy to fatigue, unwilling to move, especially after ascites, often to bed rest. Long-term bed rest, stagnation of qi and blood, is not conducive to the recovery of function. There are many ways of activity, especially walking is the most simple and suitable. I advocate cirrhosis patients to walk not less than two hours a day or ten miles a day, morning and evening once, in order not to make excessive fatigue can be intermittent. (C) Yi emotional: cirrhosis of the liver is a long course of disease, not cured for a long time, resulting in patients discouraged, depressed, lack of confidence in the cure, often can not adhere to the systematic treatment. Therefore, it is necessary to persuade and help patients to establish confidence. At the same time, family members should also be advised to create a good living environment for the patient, so that he or she can be in a happy mood, and must not go against his or her will, so that he or she can be annoyed and sad, aggravating his or her condition.
