Management of intrahepatic bile duct stones spread throughout the liver

Recently, a farmer’s wife was admitted to the hospital with a CT scan showing that her liver was covered with large stones. The patient had her gallbladder removed 20 years ago due to gallstones, but she was ashamed to seek medical attention because of chronic liver distension and pain, and her family accompanied her to the hospital and asked for a surgical treatment. The surgery was very difficult, as the liver had repeated inflammation due to the stones, and the surface of the liver was severely deformed, with severe perihepatic adhesions, so I adopted the following coping strategies during the surgery: 1) resection of the left outer lobe of the liver, which was full of stones. 2) multiple resections of the right parenchyma of the liver, which was full of large stones. 2.Multiple incisions of the right hepatic parenchyma were performed for deep bile duct stone extraction, and the common bile duct was opened for stone extraction with choledochoscope. 3, Ultrasound guidance was used to find the deep stones during the operation. In order to prevent recurrence of stones in the future, the common bile duct was cut off and a bile duct jejunostomy was performed. The surgery was finally completed, but the sad thing is that the surgery took too long, 7.5 hours !!!! The surgery took 7.5 hours, more than any other major surgery in the past. I am glad that through my efforts, the patient finally relieved, lamented: any bile duct stone if the timely medical treatment and surgery, will not develop into today’s difficult to deal with the situation.
