Case: A middle-aged male patient who was treated with surgical resection for left hepatocellular carcinoma in January 2013 and underwent hepatic artery cannulation chemoembolization (referred to as TACE) one month after surgery. 5 months after surgery, a recurrent foci were found in segment VI of the right lobe of the liver, so TACE and anhydrous alcohol intratumoral injection (referred to as PEIT procedure) were performed. two more PEIT procedures were performed in January 2014 due to reissue, and the third reissue was performed in August 2014 In early November 2014, he came to our hospital for a follow-up liver enhancement MRI, which revealed a possible localized active foci in the VIII segment of the right anterior lobe of the liver. The patient refused to continue PEIT and opted for herbal treatment. At the end of March 2015, the patient came to our hospital for follow-up MRI of the liver, and no active foci were found in the liver. Analysis: The patient recurred 5 months after surgery, and TACE and PEIT were performed several times after the recurrence, but the recurrence still occurred in the liver, why? Firstly, liver is a soil, when cirrhosis and hepatitis B exist, then the liver is more suitable for cancer cell growth, which is the basis of patient’s multiple recurrence; secondly, surgery, TACE and PEIT are all local treatment methods, surgery removes the visible cancer cells, but the invisible cancer cells remain and become the root cause of recurrence; moreover, surgery does not improve the liver Moreover, surgery does not improve the liver soil, that is, the liver soil is still suitable for the growth of cancer cells; TACE not only cannot completely destroy cancer cells, but also deteriorates the liver soil because of chemotherapy drugs and embolic agents, making it more suitable for the growth of cancer cells; PEIT also has no improvement effect on the liver soil. Therefore, for this situation, the treatment should focus on improving the liver soil to make it unsuitable for cancer cells to rise. Local treatment mainly refers to surgery, TACE, PEIT, radiotherapy, etc. to eliminate most of the cancer cells, while the remaining small portion of cancer cells can be eliminated by systemic treatment with drugs. Yin and Yang balance and acid-base constitution, with a variety of effects. The patient chooses Chinese medicine treatment, which has few side effects, is cheap and has a systemic regulating effect.
