Is surgery necessary for varicose spermatocele

  Varicocele is a common disease in young men. According to statistics, the prevalence of varicocele in the male population is 8-23%, while 40% of infertility in men has this disease. Having varicocele has certain harmful effects on the human body.  In summary, there are several aspects: 1. varicose veins affect the blood circulation of the testes, resulting in insufficient nutrition and oxygen supply to the testes, which affects the occurrence of sperm; 2. local temperature increases (the temperature of varicose veins is about 2 degrees higher than the normal temperature), which inhibits the occurrence of sperm; 3. the retention of toxic substances in the blood stimulates the testes, resulting in premature shedding of immature sperm; 4. the formation of varicose veins The internal pressure of the veins around the testes rises, which affects the discharge of metabolites; 5, causing endocrine disorders in the testes, so that the level of testosterone hormone secreted by the interstitial cells of the testes decreases, which inhibits the spermatogenic function of the varicocele and reduces the number of sperm; 6, causing testicular atrophy, and the testicular atrophy of varicocele is also obvious; 7, causing epididymal insufficiency, which reduces sperm motility.  It can be seen that varicocele mainly affects male fertility.  According to the degree of varicoceles, there are 4 levels: 1. asymptomatic (level 0): the varicoceles are not easily palpable and can only be measured with the help of instruments; 2. mild (level I): the varicoceles are not easily palpable and can only be felt by using the method of vanesse (asking the patient to pinch his nose and hold his breath downward); 3. moderate (level II): the varicoceles are not visible but can be felt; 4. severe (level III). Not only visible but also palpable trailing coiled vein mass.  Any grade 1.2 varicocele should be treated surgically. Grade 1 varicocele with recurrent prostatitis and seminal vesiculitis or with testicular shrinkage and soft texture should also be considered for surgical treatment, in addition, undocumented or mild varicocele that is not treated with medication can also be treated surgically. About 50% of patients have improved sperm quality after surgery, and 30% have achieved fertility.
